清远市清新区农村居民慢性病经济负担调查分析

Investigation and analysis on economic burden of chronic diseases among rural residents in Qingxin District of Qingyuan City

目的掌握清新区农村居民慢性病经济负担现状,为降低农村居民的慢性病经济负担提供理论依据。方法 2012年2—9月清远市疾病预防控制中心采用多阶段整群随机抽样方法,共抽取清新区206个村(居)委会,4 020户户籍人口,10 581人,进行农村居民慢性病经济经济负担现状的分析。结果当前脑血管疾病(1.42%)、糖尿病(1.37%)、心血管系统疾病(1.29%)等慢性非传染性疾病已经成为清新区农村居民的最主要疾病;清新区的慢性病主要以医院层面的治疗为主,最近1次住院的慢性病患者中接近1/2是在县级医院,住院天数在15 d及以上的占40.12%,住院治疗慢性病患者主要以每年一二次为主;患者需要多次到医院就诊、买药,大大增加了患者的医疗负担,这也是导致费用支付高、医保资金压力大的主要原因。结论慢性病给该区家庭、患者和社会造成的直接经济负担日益严重,高比医疗成本费是其重要的影响因素。

[ Objective] To know the status of economic burden caused by chronic diseases among rural residents in Qingxin District, provide theoretical basis for reducing the economic burden of chronic diseases in rural residents. [ Methods ] By using multi-stage cluster random sampling, Qingyuan Center for Disease Control and Prevention have collected 10 581 people from 4 020 households of 206 village committees in Qingxin District from February to September 2012, to analyze the status of economic burden caused by chronic diseases among rural residents. [ Results ] The chronic non-communicable diseases, including cerebrovascular disease （ 1.42% ）, diabetes （ 1.37% ） and cardiovascular system diseases （ 1.29% ）, have become the most important diseases among rural residents in Qingxin District. Most of patients with chronic diseases in Qingxin District were treated in the hospitals. Nearly half of inpatients with chronic diseases were treated in county-level hospitals, 40.12% have stayed in the hospitals for 15 days and above. The majority of patients with chronic diseases have been hospitalized once or twice every year. The frequent treatment and drug purchase had greatly increased the medical burden of the patients, and this was the main reason of expensive medical cost and high pressure of medical insurance fund. [ Conclusion ] The direct economic burden caused by chronic diseases is increasingly serious to the family, patient and society in Qingxin District, and the high proportion of medical costs is the important influencing factor.
