纤维蛋白胶复合骨形态发生蛋白与VEGF修复兔桡骨缺损的实验研究 被引量:3

摘要 目的探讨纤维蛋白胶(FS)复合骨形态发生蛋白(BMP)与血管内皮生长因子(VEGF)构建的可注射型组织工程骨修复兔桡骨缺损的作用。方法将24只新西兰大白兔在双侧桡骨骨干处建立10 mm骨缺损的动物模型。然后,严密缝合皮下组织及皮肤。按随机数字表法分为A、B、C 3组,各组8只。A组(FS+VEGF+BMP)、B组(FS+BMP)、C组(FS)将各组材料经正常皮肤注射到骨缺损处。分别于术后6、12周切取标本,进行组织形态学、X线摄片、生物力学测试等方法观察各组新骨形成的情况。结果组织学观察结果显示:术后6周,A组新生组织周围见新生血管丰富,纤维组织下有大量新生骨组织,部分区域有髓腔样结构形成;B组新生组织为纤维组织充填,新生血管不如A组丰富,骨细胞较多,骨组织较少;C组新生组织为纤维组织结构,成纤维细胞丰富,未见成骨。术后12周,A组为成熟骨组织结构,骨髓腔贯通,可见造血细胞;B组骨组织成熟度较A组稍差,髓腔结构不完整;C组仍以纤维结缔组织为主。X线摄片示:术后12周,A组骨缺损完全修复,B组骨缺损基本修复,C组无骨性愈合。X线阻射密度测量示:术后6、12周,骨缺损修复的X线阻射密度值均为A组>B组>C组(P<0.05)。生物力学测试示:术后12周,A组最大抗折载荷明显高于B组(P<0.05)。C组无骨性愈合标本,未行生物力学检测。结论 FS+VEGF+BMP及FS+BMP复合物均能不同程度促进骨组织生长及加速骨缺损的修复,VEGF能明显促进骨缺损部位早期血管化及新骨形成,比单纯应用BMP能更好地修复骨缺损,说明VEGF+BMP对促进骨生长具有协同作用。
