肾实质性高血压的诊治新进展

肾实质性高血压是最常见的继发性高血压,其发病率仅次于原发性高血压。常见的临床类型是原发性慢性肾小球肾炎或肾病综合征以及继发性肾小球疾病,常见的病理类型是系膜增生性肾炎和局灶增生或局灶硬化性肾炎。它的眼底病变和心血管并发症更严重,更易进展为恶性高血压,预后比原发性高血压更差。肾实质性高血压反过来也会危害肾脏,加快肾实质疾病进展,形成恶性循环。因此,肾实质性高血压必须积极治疗。

Renal hypertension is the most common secondary hypertension,with the incidence only after primary hypertension.Common clinical type is primary chronic glomerulonephritis or nephrotic syndrome and secondary glomerular disease;common pathological type is mesangial proliferative glomerulonephritis and focal hyperplasia or focal hardening nephritis.The fundus lesions and cardiovascular complications are more serious,which may easily develop into malignant hypertension,and the prognosis is worse than primary hypertension.Renal hypertension,in turn,will harm kidney,accelerate disease progression of renal parenchyma,forming a vicious cycle.Therefore,positive treatment must be given to renal hypertension.
