摘要 目的探讨感觉统合训练对对脑损伤鼻饲患儿吞咽功能恢复的影响。方法 2011年1月至2012年1月,选择在南方医科大学珠江医院住院治疗的脑损伤鼻饲患儿60例,按随机数字表法将其分为观察组及对照组各30例,对照组采取传统的吞咽功能训练加神经肌肉电刺激,治疗频率是1次/d;观察组在对照组的基础上增加感觉统合训练,频率是鼻饲前4次/d。比较两组患儿的平均留置胃管天数、吞咽功能恢复情况及呕吐和反流、胃潴留、呛咳、吸人性肺炎等并发症的发生率。结果观察组采取感觉统合训练后,吞咽功能恢复总有效率为93.3%,明显优于对照组的73.3%(χ2=4.32,P<0.05);同时,与对照组相比,观察组的留置胃管时间缩短(t=23,P<0.05),并发症的发生减少(χ2=5.19,P<0.05)。结论感觉统合训练对脑损伤鼻饲患儿吞咽功能恢复有积极影响,能有效促进脑损伤鼻饲患儿吞咽功能恢复,有效降低医疗费用,值得临床推广应用。 Objective To explore the effects of sensory integration training on recovery of the swallowing function for nasal feeding children with brain injury. Methods Totally 60 nasal feeding children with brain injury were randomly divided into control group and observational group. Children in both groups received conventional swallowing behavior therapy and neuromuscular electrical stimulation once a day,while children in the observational group were given additional sensory integration training four times a day before nasal feeding. Days of stomach tube indwelling, recovery of swallowing function, incidence of vomiting and reflux,gastric retention, choking, aspiration pneumonia and other complications were compared between the two groups. Results Total effective rate of swallowing function recovery for the observational group (93.3%) was significantly better than that in the control group (73.3%)(x^2=4.32,P〈0.05),time for tube indwelling was significantly shorter than the control group(t=23,P〈0.05),incidence of complications was significantly less than the control group(x^2=5.19, P〈0.05). Conclusion Sensory Integration training can promote swallowing function recovery of nasal feeding children with brain injury and reduce medical costs ,which is worthy of further clinical application.
