铺棉灸疗法缓解带状疱疹疼痛的临床研究

Clinical Research on Pain Relief by Thin Cotton Sheet Partition Moxibustion and Acupuncture in the Treatment of Herpes Zoster

【目的】评价以铺棉灸疗法为主针灸治疗对带状疱疹疼痛的影响。【方法】将2008年1月至2009年8月在成都、武汉、广州9个研究中心收治的201例带状疱疹患者,随机分为铺棉灸组99例和西药组102例。铺棉灸组给予铺棉灸为主针灸治疗,西药组给予常规西药治疗。2组均以10 d为1个疗程,1个疗程后观察疗效,并于研究期间22 d、30 d、60 d、90 d对患者进行随访。【结果】(1)铺棉灸疗组总有效率为92.9%,西药组为91.2%,差异无统计学意义(P>0.05);(2)视觉模拟评分(VAS)方面,2组干预前后及组间比较,差异均有统计学意义(P<0.01);(3)疼痛持续时间方面,铺棉灸组与西药组比较,差异有统计学意义(P<0.01)。(4)后遗神经痛发生情况方面,第30日铺棉灸组与西药组比较,差异有统计学意义(P<0.01)。【结论】以铺棉灸疗法为主的针灸治疗对带状疱疹疗效确切,与西药常规疗法无明显差异,但在镇痛及控制带状疱疹后遗神经痛发生方面,疗效优于西药疗法。

Objective To observe the efficacy of thin cotton sheet partition moxibustion plus acupuncture on relieving pain of herpes zoster patients. Methods Two hundred and one herpes zoster patients admitted in nine research centers of Chengdu, Wuhan and Guangzhou cities from January of 2008 to August of 2009, were randomly divided moxibustion group （N=99） and western medicine group （N=102）. The moxibustion group was given thin cotton sheet partition moxibustion plus acupuncture, and the western medicine group was given conventional western medicine. Results （ 1 ） The total effective rate of moxibustion group was 92.9%, and was 91.2% in the western medicine group, the difference being insignificant between the two groups （P〉0.05）. （2） The Visual Analog Scale （VAS） scores in both groups after treatment differed from those before treatment （P〈 0.01）, and the difference was also significant between the two groups after treatment （P〈0.01）. （3） The pain duration of moxibustion group was shorter than that of western medicine group （P〈0.01）. （4） The incidence of sequela of neuralgia 30 days after therapy was lower in the moxibustion group than that in the western medicine group （P〈0.01）. Conclusion The therapeutic effect of thin cotton sheet partition moxibustion plus acupuncture for herpes zoster patients is similar to conventional western medicine, and thin cotton sheet partition moxibustion plus acupuncture is more effective in pain relief and in lowering the incidence of sequela of neuralgia .
