多层螺旋CT三维图像重组在先天性髋关节脱位中的应用

摘要 目的探讨多层螺旋CT三维图像重组技术在儿童先天性髋关节脱位(CDH)中的应用价值。方法对先天性髋关节脱位患者47例,行双侧髋关节螺旋CT三维图像重组,测量前倾角(FNA)、髋臼指数(AI)及髋臼壁的发育情况,与术中所见比较。结果 47例患儿前倾角均增大,最大89°,最小40°,平均(42.52±11.86)°,髋臼指数均有不同程度增大,平均(35.4±9.3)°,髋臼各壁不同程度发育不良,与术中所见符合。结论髋关节多层螺旋CT三维图像重组技术可以直观、全面及分解地显示髋关节的病理形态及发育不良的程度,更加精确地测量AI和FNA,提供了一个更具真实感的三维立体影像,对术者选择适当的治疗方案及手术入路,纠正前倾角及髋臼指数均具有重要意义。 Objective To explore the value of using multi-slice spiral CT three-dimensional reconstruc- tion in diagnosing congenital dislocation of hip (CDH) in children. Methods A total of 47 patients with CDH underwent spiral CT three-dimensional reconstruction. The femoral neck anteversion (FNA) angles and the acetabular index (AI) were measured and compared with the results found in operation. Results 47 patients' FNA were all greater than normal, With the greatest being 89°, the smallest being 40°and the av- erage being 42.52°±11.86±. Their AI became greater to different degrees, with the average being 35.4° 9.3°. The acetabula showed hypogenesis to different degrees, which was consistent with the findings in operation. Conclusion The 3D CT reconstruction of hip joint provides precise information of pathological imaging, dysplastic status, AI and FNA angles and is useful for the diagnosis and treatment of CDH.
