椎-基底动脉缺血发作与睡眠呼吸障碍

VBI AND SLEEP RELATED BREATHING DISORDERS

对 45例椎 基底动脉缺血发作 (VBI)患者进行了多导睡眠图检查 ,了解其发病率和性质 ,并分析其与临床各因素的关系。结果表明 :18例 (4 0 % )并发睡眠呼吸障碍 (SRBD) ,2 / 3为阻塞性者 ,1/ 3为中枢性者 ,与肥胖有关 ,且易并发心血管病变。提示 :长期反复缺血导致的脑干功能障碍 ,有可能是VBI中SRBD ,特别是中枢性者发生的机制。SRBD继发的心律改变和高血压可加重VBI。VBI与SRBD可以互为因果 ,造成恶性循环 ,应该引起重视 ,加强SRBD的防治措施。

To discuss the relationship between vertebrobasilar ischemia attacks (VBI) and sleep related breathing disorders (SRRD). Polysomnographic studies were performed in 45 cases of VBI. The incidence and characteristics of SRBD, as well as its relationship with various clinical factors were analyzed. Results: 18 cases (40%) were complicated with SRBD, among which 2/3 was obstructive and 1/3 was central type. It was correlated with obesity and was prone to be complicated with cardiovascular disorders. Conclusions: Brainstem dysfunction caused by long term repeated ischemia is probrobly the pathogenesis of SRBD, especially its central type. While the arrhythmia and hypertension secondary to SRBD may aggravate VBI. VBI and SRBD may be a reciprocal causation and form a vicious circle. Attention should be paid to the prevention and treatment of SRBD in VBI.
