三阶梯止痛联合心理、伦理学干预对癌痛患者生活质量的影响

The Influence of Three Step Analgesia Combined with Psychological and Ethical Intervention on Life Quality of Patients with Cancer

目的探讨三阶梯止痛联合心理、伦理学干预对癌症患者疼痛状况和生活质量(QOL)的影响。方法入组癌痛病人106例,随机分为A组(三阶梯止痛治疗组)和B组(三阶梯止痛联合心理、伦理干预组),入院日和出院日对所有患者行NRS自测量表测量及EORT C QLQC30 V3.0问卷。对A、B两组患者疼痛状况和生活质量进行评估。结果 B组患者疼痛状况较A组患者改善更明显(P<0.01 vs P<0.05)并且生活质量提高更显著。结论三阶梯止痛联合心理、伦理干预较单纯药物止痛能明显改善患者疼痛状况及显著提高生活质量,这提示在今后的癌痛控制中应注重肿瘤学、心理学和伦理学等多学科联合干预。

Objective： To investigate the influence of three step analgesia combined with psychological and ethica/intervention on life quality of patients with cancer. Methods ：The 106 patients with cancer pain were randomly divided into group A （three step analgesia） and group B （ three step analgesia combined with psychological and ethical intervention） , all patients were evaluated by the NRS selftest Scale and EORT CQLQC30 V3.0 question- naire at the day of admission and discharge day. The patients pain and life quality were investigated. Results： The quality of life of group B patients with pain conditions were improved significantly than group A （P 〈0.01 vs P 〈 0.05 ）. Conclusions： Three step analgesia combined with psychological and ethical intervention can signifi- cantly improve patients＇life quality, which suggests that it should focus on oneology, psychology, and ethics of multidisciplinary combined intervention method in the future of cancer pain control.
