滋肾健脾化瘀片联合激光治疗糖尿病性黄斑水肿的疗效观察

Clinical observation on Zishen Jianpi Huayu Tablet and laser treatment for diabetic macular edema

目的观察滋肾健脾化瘀片联合激光光凝治疗糖尿病性黄斑水肿(DME)的疗效。方法 43例(86只眼)DME患者按数字随机表法分为激光组21例(42只眼)和联合组22例(44只眼),激光组予视网膜激光光凝治疗;联合组口服滋肾健脾化瘀片联合视网膜激光光凝治疗,疗程为3个月。结果视力:2组治疗后视力均有提高,治疗后激光组有效率为54.76%(23/42),联合组有效率为79.55%(35/44),联合组视力明显优于激光组,差异有统计学意义(P<0.05);OCT:2组治疗后黄斑中心小凹视网膜厚度及黄斑中心凹1 mm直径视网膜厚度平均值均较前改善,差异有统计学意义(P<0.05);FFA:激光组黄斑区荧光渗漏消失或减轻36只眼,有效率为85.71%(36/42),联合组相应的眼数为44只眼,有效率为100%(44/44),2组差异有统计学意义(P<0.05)。结论滋肾健脾化瘀片联合激光治疗比单纯激光光凝治疗DME疗效更好。

OBJECTIVE To observe the efficacy of Zishen Jianpi Huayu Tablet combined with laser photoco- agulation treatment for diabetic macular edema. METHODS Forty-three （86 eyes） macular edema patients were randomly divided into laser group of 21 cases （42 eyes） and combined group of 22 cases （44 eyes）. Laser group was given retinal laser photoeoagulation, while combined group was given oral Zishen Jianpi Huayu Tablet combined with retinal laser photocoagulation,treatment for three months. RESULTS Vision： After treatment, visual acuity improved. The total effective rate of the laser group was 54.76% （23/42）, the combined group was 79.55% （35/44）, and the difference was statistically significant （P〈0.05）. OCT： The retinal thickness of the macular center and the retinal thickness of lmm diameter of the fovea in the two groups was better than before,and the difference was statistically significant （P〈0.05）; FFA： Macular edema and leakage of the laser group disappeared or mitigated for 36 eyes, and the effective rate was 85.71% （36/42）; Macular edema leakage disappeared or reduced in 44 eyes after treatment of the combined group, and the effective rate was 100% （44/44）, the difference was statistically significant （P〈0.05）. CONDLUSIONS Zishen Jianpi Huayu Tablet combined with laser was much better than simple laser photocoagula- tion in the treatment of DME.
