摘要
目的探讨无创正压通气治疗COPD合并Ⅱ型呼吸衰竭的疗效。方法选取2010年6月-2011年10月收治的COPD合并Ⅱ型呼吸衰竭患者64例作为研究对象,将上述患者随机分为两组,观察组32例,在常规治疗的基础上给予无创正压通气治疗,对照组32例,仅给予常规治疗。比较两组患者的疗效。结果 (1)治疗后,所有患者的pH、动脉血氧分压(PaO2)显著升高,动脉二氧化碳分压(PaCO2)显著下降,治疗前后相比差异有统计学意义(P<0.05);观察组的pH、PaO2升高幅度显著大于对照组,PaCO2降低幅度显著大于对照组,两组比较差异有统计学意义(P<0.05)。(2)治疗后,所有患者的呼吸频率(RR)、心率(HR)显著降低,治疗前后相比差异有统计学意义(P<0.05);观察组的RR、HR降低幅度显著大于对照组,两组比较差异有统计学意义(P<0.05)。结论无创正压通气治疗COPD合并Ⅱ型呼吸衰竭疗效确切,该治疗技术可以在临床上推广与应用。
Objective To Explore noninvasive positive pressure ventilation treatment respiratory failure of COPD combined type I] curative effect.Methods To Select from June 2010 to October 2011, 64 patients in my department with respiratory failure of COPD combined type 1I as the research object, the patients were randomly divided into two groups, the observation group 32 cases, we based on routine treatment and give nopositive pressure ventilation treatment, 32 cases control group, we only give the routine treatment. Compared the two groups of patients curative effect.Results (1)After treatment, all patients with pH ,artery partial pressure oxygen (PaO2)significantly increased, artery carbon dioxide points pressure(PaCO 2) decreased significantly, the difference in before and after treatment was statistically significant (P〈0.05); The observation group of the pH value and the PaCO 2 increase significantly greater than the control group, PaCO 2 reduction of significant higher than the control group, the two groups of comparisons difference have statistically significant (P〈0.05). (2)After treatment, all patients with respiratory frequency (RR), heart rate (HR) was significantly lower, before and after treatment was statistically significant difference (P〈0.05); The observation group of RR HR reduction of significant higher than the control group, the two groups of comparisons difference have statistically significant (P〈0.05).Conclusion Noninvasive positive pressure ventilation treatment respiratory failure of COPD combined type I] have definite effect ,the treatment technology can be in clinical popularization and application.
