2型糖尿病患者血清铁及转铁蛋白水平与非酒精性脂肪肝的关系 被引量:8

摘要 目的研究2型糖尿病患者血清铁、转铁蛋白水平与非酒精性脂肪肝的关系。方法选取2型糖尿病合并非酒精性脂肪肝患者73例作为病例组,另选取单纯2型糖尿病患者35例作为对照组,测定两组血清铁、转铁蛋白、转铁蛋白饱和度;并应用超声定量肝脏脂肪含量技术测定肝脏脂肪含量。结果病例组血清铁、转铁蛋白、转铁蛋白饱和度分别为(18.04±5.05)μmol/L、(2.27±0.35)g/L和(36.02±12.08)%,对照组分别为(14.54±5.24)μmol/L、(2.10±0.51)g/L和(31.41±11.93)%。与对照组比较,血清铁在病例组增高,差异有统计学意义(P<0.05);血清转铁蛋白、转铁蛋白饱和度两组间差异无统计学意义(P>0.05);二元Lo-gistic回归分析显示,血清铁是非酒精性脂肪肝的危险因素。将肝脏脂肪含量等分为四分位组(Q1~Q4),Q1组血清铁水平(14.09±4.89)μmol/L,Q2组(14.15±4.70)μmol/L,Q3组(18.97±5.38)μmol/L,Q4组(20.21±5.40)μmol/L,Q1组与Q3组、Q4组间差异有统计学意义(P<0.05)。结论 2型糖尿病患者中,血清铁是非酒精性脂肪肝的危险因素,可作为诊断非酒精性脂肪肝的重要参考指标;随着肝脏脂肪含量升高,血清铁的水平增高。
