原发性头痛151例临床分析及发病机制研究

Clinical Analysis on 151 Patients with Primary Headache and Its Pathogenesis

目的探讨原发性头痛临床诊治特点及发病机制。方法分析本病患者临床诊治资料,并对30例偏头痛相关神经肽和内分泌激素进行检测。结果偏头痛、紧张性头痛、丛集性头痛临床头痛控制率分别为81.3%、91.5%、71.9%,总有效率为91.7%、96.4%、82.1%,差异有统计学意义(P<0.05);偏头痛组NPY量值显著低于正常人组(P<0.01),而B-EP量值高于正常人组(P<0.05);偏头痛组AT-Ⅱ和ET量值均显著高于正常人组(P<0.01)。结论原发性头痛症状特点明显,其发病机制与神经血管功能改变,包括神经递质、血管活性肽等密切相关。

Objective To investigate the clinical characteristics and its pathogenesis of primary headache. Methods The clinical data of diagnosis and treatment of 151 patients with primary headache were analyzed. Nerve peptide and endocrine hormone of 30 ca- ses with migraine were examined. Results Clinical control rate of migraine, tensional headache and cluster headache were 81.3%, 91.5 %, and 71.9 %, respectively. The total effective rate were 91.7 %, 96.4 % and 82. 1%, respectively （P〈0.05）. The neuropeptide Y （NPY） value in migraine group was significantly lower than that in normal people group （P〈0. 01）. The beta - endorphin （B- EP） val- ue in migraine group were higher than hat in normal people group （P〈0.05） ;The angiotensin Ⅱ （AT -Ⅱ） and endothelin - 1 （ET - 1） val- ue in migraine group was higher than that in normal group （P〈0.01）. Conclusion Primary symptomic headache had special charac- teristics,its pathogenesis was closely related to the neurotransmitters and vasoactive peptides.
