彩色多普勒超声定性诊断阴囊肿块89例分析

Color doppler ultrasound in the qualitative diagnosis of scrotal mass

目的探讨阴囊肿块的超声声像图特征及彩色多普勒超声在阴囊肿块定性诊断中的价值。方法对89例阴囊肿块进行彩色多普勒超声检查,总结不同病理类型阴囊肿块的声像图特征,并与术后病理对照。结果在89例阴囊肿块中,彩超正确筛出80例(89.89%)。良性肿块39例,其中睾丸表皮样囊肿可呈现特征性"洋葱皮"样及"蛋壳"样回声,未见血流信号;睾丸畸胎瘤多表现为混合回声,内部见钙化灶及液性暗区,肿块内有少量血流信号;睾丸附睾扭转显示睾丸回声低弱不均匀、附睾回声增高杂乱,未见血流信号或有伪像血流;附睾腺瘤样瘤肿块呈稍低回声,边界清楚,血流信号不丰富;附睾炎性病变超声表现为附睾轻中度肿大,回声杂乱不均,血流信号丰富。睾丸恶性肿瘤50例,超声表现为睾丸形态失常、体积增大、肿块呈实质性低回声或不均匀混合回声,边界不清,肿块周边及内部血流信号丰富,为粗大低阻动脉血流。结论高频超声及彩色多普勒超声可明确判定阴囊肿块的部位。阴囊多数良恶性肿块高频二维超声有其较特征性声像图表现,结合彩色多普勒血流显像绝大多数可以明确诊断其良恶性,可作为阴囊肿块及急诊的首选诊断方法,有较高的临床应用价值。

Objective To explore the sonographie characteristics of different types of scrotal mass and the value of color Doppler ultrasound（CDUS） in qualitative diagnosis of scrotal mass. Methods 89 eases of scrotal mass is given CDUS inspection, re- view the sonographic features of different pathological types scrotal mass. Control with pathologic results, in order to assess the CDUS on the accuracy of the diagnosis of scrotal mass. Results Among 89 cases of scrotal mass, 80 cases were correctly diagnosed by ultra- sound, the accuracy rate is 89. 89%. There were 39 benign lumps and 50 testicular cancer. Ultrasound showed the features of testicular epidermoid cyst were＂ onion-like＂ ,testicular teratoma were mixed echo; testicular torsion were no blood flow signal or artifacts blood flow .epididymal adenomatoid tumor masses were slightly lower echo, epididymitis were hypoechoic or inhomogeneous mixed echo and a- bundant blood flow. Conclusions CDUS plays an important clinical role in the differential diagnosis of scrotal mass.
