经皮加压钢板与防旋股骨近端髓内钉内固定治疗老年性股骨粗隆间骨折的对比分析 被引量:12

摘要 目的对经皮加压钢板(percutaneouscompressionplatingsystem,PCCP)和防旋股骨近端髓内钉(proximalfemoralnailantirotation,PFNA)两种内固定治疗老年性股骨粗隆间骨折患者进行对比分析。方法2009年1月至2012年9月我院收治并得到随访的48例老年股骨粗隆间骨折患者,分别采用PCCP和PFNA进行治疗,从手术时间、术中出血量、术中x射线暴露时间、术后髋关节Harris法评分、术后并发症几个方面进行分析比较。结果全部病例均获随访5~19个月,两组术中出血量、术中X射线暴露时间比较差异有统计学意义(P〈0.05),在手术时间、术后髋关节功能评分、术后并发症方面比较差异无统计学意义(P〉0.05)。结论PCCP、PFNA两种内固定方式均是治疗老年骨质疏松性股骨粗隆间骨折的较合理有效的方法,且各有不同优势。 Objective To compare the effect of two kinds of internal fixations in treatment of intertrochanterie hip fraetures in elder patients. Methods Forty-eight elder patients with intertrochanterie hip fractures from January,2009 to June,2012 were followed up. They were treated with the two kinds of operations respeetively:pereutaneous compression plating system(PCCP) or proximal femoral nail antirotation(PFNA). Operative time,blood loss,fluoroseopy exposures,functional outcome(Harris Hip Score) ,intrraoperative com- plications were statistically compared. Results The differences of blood loss and fluoroscopy exposures between the two methods were statistically signifieant(P 〈 0. 05). No differences of functional outcome( Harris Hip Score) ,intrraoperative complications and opera-tive time were found between the two techniques. Conclusion PCCPand PFNA all are effective internal fixation methods for intertrochanteric hip fractures. Each has different advantages.
