中西医治疗慢性肛周湿疹120例临床观察

Clinical Observation on Theatment of 120 Cases Chronic Perianal Eczema with Integrated Traditional and Western Medicine

目的观察中西医联合治疗慢性肛周湿疹的临床疗效和安全性。方法采用随机数字表法将入选的120例慢性肛周湿疹患者随机分为2组,各60例。两组患者均予复方亚甲蓝注射液在肛周局部点状封闭治疗,治疗组予湿疹1号(组分:黄芪、山药、乌稍蛇、酸枣和夜交藤各30g,当归、生地黄和野菊花各20g,白芍、栀子和丹皮各15g,蝉蜕和川芎各10g,甘草6g)内服和湿疹2号(组分:苦参、土茯苓、川椒、大黄、紫草、野菊花和金银花各30g,生百部、白鲜皮、蛇床子、地肤子、黄柏、生地榆和枯矾各20g,五倍子、丹参和薄荷各15g,冰片6g)熏蒸并坐浴治疗;疗程均为2周。结果治疗结束后,两组患者的瘙痒、渗液、皮损等临床症状积分均较治疗前降低,生活质量评分较治疗前升高,且治疗组的疗效和临床症状恢复正常的时间优于对照组,以上差异均有统计学意义(P均<0.05)。两组患者也均未见严重不良反应。结论中西医联合疗法治疗慢性肛周湿疹治愈率高,疗程短,且复发率低,安全性高。

Objective To observe the clinical efficacy and safety of combined treatment of traditional Chinese and Western medicine for chronic perianal eczema. Methods One hundred and twenty patients with chronic perianal eczema were randomly divided into two groups, each of 60 cases, by a random digits table method. Two groups of patients are both given compound methylene blue injection at the local point-like closure of crissum. Treatment group were given eczema-lst for oral use and eczema-2nd for fumigation and bath treatment for two weeks. The components of eczema-1 st are： Astragalus, Chinese yam, black little snake, jujube and tuber fleeceflower stem 30g each; Angelica, Rehmannia and wild chrysanthemum 20g each; white peony, gardenia and cortex moutan 15g each; cicada Slough and Chuanxiong 10g each; licorice 6g. The components of eezema-2nd are： kuhseng, glabrous greenbrier rhizome, Sichuan pepper, rhubarb, comfrey, wild chrysanthemum and honeysuckle 30g each, fresh stemona root, cortex dietamni, Cnidium, fruetus kochiae,amoorcorn tree bark, raw Burnet and alum 20g each; gall, danshen root and mint 15g each; borneo1 6g. Results After treatment, the clinical symptom （ pruritus, exudate, skin lesions and other） scores of the two groups were lower than that before treatment; the quality of life scores were higher than before treatment ; and the efficacy of the treatment and the recovery time of clinical symptoms were better than control group, these differences were statisticaUy significant （ P 〈 0.05 ）. No serious adverse reactions were found in the two groups of patients. Conclusion Combined treatment of traditional Chinese and Western medicine for chronic perianal eczema has the advantage of high cure rate, short course of treatment, lower recurrence rate and high security.
