摘要 目的探讨细胞间黏附分子1(ICAM-1,CD54)在儿童肿瘤中的阳性表达率及其临床意义。方法选择2008年1月1日-2012年12月31日收治的儿童肿瘤患者共85例和对照组20例。ICAM-1阳性表达,实体瘤35例采用免疫组织化学方法检测;白血病50例和对照组20例均采用流式细胞仪检测。比较白血病与对照组ICAM-1阳性表达率差异。结果实体瘤中肝母细胞瘤ICAM-1检出100.0%(2/2),其他检出ICAM-1的实体肿瘤有淋巴瘤、横纹肌肉瘤、神经母细胞瘤、尤文氏肉瘤,而肾母细胞瘤和原始神经外胚层肿瘤未检出ICAM-1;急性淋巴细胞白血病和急性髓系白血病检出率分别为50.0%(10/20)和53.3%(16/30)。急性白血病ALL患儿骨髓细胞的ICAM-1阳性表达率为50.0%,与对照组比较无显著差异(P>0.05);AML(M1、M2和M3)组ICAM-1阳性表达率为66.7%,显著高于对照组10.0%(2/20)(P<0.05);AML(M4、M5)组ICAM-1阳性表达率为33.3%,显著低于对照组75.0%(15/20)(P<0.05)。结论儿童肿瘤中ICAM-1表达存在差异,肝母细胞瘤、白血病患儿ICAM-1阳性率较高,临床上治疗时应考虑优先使用CIK细胞免疫治疗。 Objective To investigate expression and clinical significance of intereellular adhesion molecule-1 ( ICAM-1 , CD54) in pediatric tumor. Methods 85 pediatric tmnor patients and 20 cases of the control group were selected,ICAM-1 of 35 cases of solid tumors were detected using immunohistochemistry;ICAM-1 of 50 cases of leukemia and 20 control cases were detec- ted using flow cytometry and then compared the difference of ICAM-1 levels between leukemia patients and control cases. Results The ICAM. 1 detection rate of Hepatoblastoma tumor was 100.0% (2/2) , ICAM-1 in rhabdomyosareoma,neuroblastoma, Ewingg sarcoma, Wilms tumor were also detected, the ICAM-I detection rate of acute lymphoblastic leukemia and acute myeloid leukemia were 50.0% (10/20) and 53.3% (16/30). compared with the control group,ICAM-1 positive expression rate of Acute leukemia in children was 50.0% , there was no significant difference ( P 〉 0.05 ) ;AML ( M1, M2 and M3 ) group was 66.7% , significantly higher than 10.0% (2/20) in the control group ( P 〈 0.05 ) ; AML ( M4, M5 ) group was 33.3%, significantly lower than 75.0% (15/20) in the control group, ( P 〈 0.05 ). Conclusion The ICAM-1 levels in pediatric tumors are different,its positive expres- sion rate in hepatoblastoma and leukemia patients arc high. CIK cells immunotherapy should be preferred choice.
