紫杉醇联合顺铂化疗序贯放疗局部晚期食管癌的临床观察

Clinical Observation of Radiotherapy Combined with Chemotherapy of Paclitaxel and Cisplatin for Local Advanced Nasopharyngeal Carcinoma

目的观察紫杉醇联合顺铂化疗序贯放疗对局部晚期食管癌的治疗效果及毒副作用。方法选取我院收治的局部晚期食管癌患者60例作为研究对象,随机分为对照组(单纯放疗)和治疗组(紫杉醇、顺铂化疗联合放疗),每组30例。比较2组患者治疗效果和毒副作用情况。结果对照组有效率为66.7%,治疗组有效率为83.3%,治疗组有效率显著高于对照组(P<0.05);治疗组放射性肺炎、恶心呕吐、放射性食管炎发生率略高于对照组,但2组之间比较差异不显著(P>0.05)。对2组患者进行为期1年的随访,对照组患者3个月生存率为63.3%,6个月生存率为36.7%,1年生存率为20.0%;治疗组3个月生存率为76.7%,6个月生存率为53.3%,1年生存率为43.3%。治疗组患者生存率显著高于对照组(P<0.05)。结论对于确诊无法采取手术治疗的局部晚期食管癌患者进行紫杉醇联合顺铂化疗序贯放疗能显著提高治疗效果和患者生存率,且毒副作用未见有加重。

Objective To evaluate the therapeutic effects and side effects of paclitaxel, cisplatin chemotherapy followed by radiotherapy for locally advanced esophageal cancer. Methods 60 patients with advanced esophageal carcinoma were random- ly divided into the control group（ only radiotherapy） and the treatment group （paclitaxel, cisplatin combined with radiotherapy）, each group had 30 cases. The treatment effects and the side effects were compared. Results The effective rate of the control group and the treatment group was 66.7% and 83.3%. The effective rate of the treatment group was significantly higher than that of the control group （ P 〈 O. 05 ）. The incidence rate of radiation pneumonitis, radiation esophagitis, and nausea＆vomiting in treat- ment group was slightly higher than that in the control group, but the difference between the 2 groups was not significant （P 〉 O. 05 ）. 3-month ,6-month, 1-year survival rate of the patients in the control group was 63.3%, 36.7% ,20% respectively and in the treatment group was 76.7% ,53.3% ,43.3% respectively. The survival rate of the patients in treatment group was significant- ly higher than that in control group（ P 〈 0.05 ）. Conclusion Paclitaxel, cisplatin chemotherapy followed by radiotherapy for lo- cally advanced esophageal cancer can significantly improve the therapeutic effect and survival rate without increasing side effects.
