异位妊娠孕囊与黄体囊肿的超声鉴别

Sonographic comparison of ectopic gestational sac with corpus luteum cyst

目的对异位妊娠孕囊与早孕时卵巢黄体囊肿超声特点进行比较,以提高异位妊娠的早期检出率。方法回顾分析异位妊娠(孕囊型)79例和早孕时卵巢黄体囊肿84例的经阴道超声图像特点,记录异位妊娠孕囊及黄体囊肿大小、灰阶超声图像、彩色血流特点及彩色血流的阻力指数。结果异位妊娠孕囊平均大小(1.2±0.5)cm,黄体囊肿平均大小(2.1±0.7)cm,黄体囊肿有4种类型;有81.01%的异位妊娠孕囊囊壁回声高于卵巢实质回声,而82.14%的黄体囊肿囊壁回声等于或低于卵巢实质回声(P<0.01);黄体囊肿组囊壁血流阻力指数较异位妊娠孕囊低(P<0.05)。结论异位妊娠孕囊与卵巢小黄体囊肿可通过比较囊壁回声、血流阻力指数等超声资料及结合临床资料相鉴别。

Objective： To compare the sonographic characteristics of ectopic gestationai sac with corpus luteum cyst in order to increase the earlier detection rate.Methods： Sonograms in 79 cases with unruptured ectopic pregnancy and 84 cases with the corpus luteum were retrospectively reviewed.The size of ectopic gestationai sac and corpus luteum cyst,gray scale ultrasound,the character of blood flow and resistent index were recorded.Results： The average sizes were（1.2 ± 0.5） cm in ectopic gestation sac and（2.1 ± 0.7） cm respectively.There were four types in the corpus luteum cyst.The walls of ectopic gestation sac were more echogenic than those of the corpus luteum（P 0.01）.The resistent index in corpus luteum was lower than that in ectopic gestation sac（P 0.05）.Conclusion： Capsule wall resonance and resistent index combined with the clinical data contribute to the differential diagnosis between ectopic gestational sac and corpus luteum cyst.
