激光辅助孵化对冷冻胚胎移植和新鲜胚胎移植患者临床妊娠结局的影响 被引量:6

摘要 目的:探讨激光辅助孵化(LAH)对冷冻胚胎移植患者及新鲜胚胎移植患者临床妊娠结局的影响。方法:选取2010年12月至2012年4月于郑州大学第三附属医院生殖医学中心接受辅助生殖技术治疗的患者。冷冻胚胎移植患者184个周期,其中152个周期行LAH(LAH组),32个周期未行LAH,常规处理(对照组)。新鲜胚胎移植患者461个周期,其中<35岁行LAH共36个周期,不行LAH333个周期;≥35岁行LAH共70个周期,不行LAH22个周期。随访以上患者的妊娠情况。结果:LAH对冷冻胚胎移植患者的临床妊娠率、多胎率、种植率和流产率无影响(P均>0.05)。对于年龄<35岁的新鲜胚胎移植患者,是否行LAH对临床妊娠率、多胎率、种植率和流产率无影响(P均>0.05);对于年龄≥35岁的新鲜胚胎移植患者,行LAH的患者临床妊娠率和种植率均高于未行LAH的患者(χ2=4.234、3.939,P均<0.05)。结论:LAH仅可以提高高龄患者新鲜胚胎移植的临床妊娠率和种植率。 Aim: To investigate the effects of laser-assisted hatching (LAH) on the clinical outcomes of IVF-ET.Methods: The patients undergoing IVF /ICSI in our center were collected from December 2010 to April 2012.The patients accepted frozen embryo implantations(184 cycles) were allocated into LAH group(152 cycles) and control group(32 cycles) .The patients accepted fresh embryo implantation (461 cycles) were allocated into 2 groups according to the patients' age,that was ≥35 years group(92 cycles) and35 years group(369 cycles) ,and they were further allocated into LAH group and control group.Pregnant outcomes were compared between these groups.Results: For frozen embryo implantation patients,there was no significant difference between LAH and control groups in the rates of implantation,clinical pregnancy,multiple pregnancy and abortion(P0.05) .For fresh embryo implantation patients,in35 years group,the rates of implantation,clinical pregnancy,multiple pregnancy and abortion between LAH and control subgroups were similar(P0.05) ; in ≥35 years group,the rates of implantation and clinical pregnancy in the LAH subgroup were higher than those in control(χ2=4.234,3.939,P0.05) .Conclusion: LAH improves the implantation rate and clinical pregnancy rate in patients older than 35 years during IVF-ET using fresh embryos.
