最终消融靶点位于左后间隔的B型预激综合征患者体表心电图的特征

摘要 目的:探讨体表心电图提示右后间隔旁道,而最终消融靶点在左后间隔的预激(WPW)综合征患者体表心电图特征。方法:WPW综合征患者37例,经心内电生理标测到最佳消融靶点位于左后间隔(A组)9例,右后间隔(B组)28例;体表心电图的监测项目包括:①δ波40ms时方向。②V1导联δ波时间。③V1导联波型(rS或QS)。④Ⅱ、V1~V6导联的T波方向。结果:A组V1~V6导联T波正向的个数多(t=3.423,P<0.001),V1导联呈rS波型多(P=0.024),患者首次发作室上性心动过速的年龄较大。结论:在B型WPW综合征患者的体表心电图上,V1~V6导联T波正向的个数多、V1导联呈rS波型,左后间隔旁道的可能性大。 Aim: To find out the features of surface electrocardiogram(ECG) firstly suggesting right posterior septal(RPS) accessory pathway in patients with Wolff-Parkinson-White(WPW) syndrome,but final ablation target at left posterior septal(LPS) .Methods: A total of 37 patients were divided into LPS(group A,n=9 ) and RPS (group B, n=28 ) by radiofrequency catheter ablation.The parameters of surface ECG examination included δ wave's direction at 40 ms; δ wave's time in V1; wave shape in V1(rS or QS) ; T wave's direction in Ⅱ,V1~V6.Results: Compared with group B,T wave positive in V1~V6 and rS wave in V1 in group A were significantly more(P=0.024 ) .Conclusion: In surface ECG of patients with type B WPW syndrome,more T wave positive in V1~V6 and rS wave in V1 suggest larger probability of LPS.
