急性脑外伤合并高血压患者降压幅度与疗效的关系分析

Analyze Cute Cerebral Injury Combined with Hypertension Patients' Depressurization Range's Relationship with Curative Effect

目的:分析急性脑外伤合并高血压患者降压幅度与疗效的关系。方法:选取我院90例急性脑外伤合并高血压患者,根据治疗后的血压值与其基础血压值的差值进行比较分组,总分为9组,研究分析血压控制程度与疗效的关系。结果:所有脑外伤患者急性期血压均有不同程度的升高,各组降压幅度不同临床治疗效果也不同,其中疗效最显著的为第8组,即血压控制后与原先的基础血压值差值收缩压为0~20mmHg,舒张压为0~10mmHg。第8组伤后1周CT显示损伤灶体积扩展不明显,显著小于其它组患者,而且格拉斯哥昏迷评分(GCS)明显改善。结论:急性脑外伤合并高血压患者早期进行降压治疗十分重要,降压幅度要根据基础血压值而定,不宜过高也不宜过低,收缩压控制在高于基础血压值0~20mmHg和舒张压高于基础血压值0~10mmHg时疗效最佳。

Objectives：To analyze cute cerebral injury combined with hypertension patients＇ dcpressudz^tion range＇s relationship with curative effect.Methods：Chose 90 cases of cute cerebral injury combined with hypertension patients who have been treated at our hospital as clinical study objectives,divided them into nine groups according to their pressure values after treatment and their differences with basic blood pressure value,studied and analyzed blood pressure control degree＇s relationship with curative effect.Results：All patients＇ blood pressure had different degree of increasing,each group had different depressufization range,and their clinical treatment effect was different too,group eight＇s curative effect was the most significant, their systolic prassure（difference between the basic blood pressure value and the value after treatmen0 was 0~20 mmHg, diastolic pressure was 0-10 mmHg.Computed tomography showed that group eight＇s patients＇ damaged volume did not expand obviously after one week of treatment,their damaged volume was evidently less than other groups＇ patients＇,and their Glasgow coma scale improved a lot. Conclusions：It is very important to proceed blood-pressure-lowering treatment to these cute cerebral injury combined with hypertension patients,the depressurization range should base on basic blood pressure value,it should not be too high or too low,when the systolic pressure is 0-220 mmHg higher than basis blood pressure value and the diastolic pressure is 0-10 mmHg higher than basis blood pressure value ,the curative effect is the best.
