摘要
目的:探讨CT引导下经皮肺穿刺术与电子支气管镜检查在不明原因肺部病变中的临床应用。方法:选取我院自2009年9月~2012年9月收治的86例不明原因的肺部疾病患者作为临床研究对象。86例患者分为A、B两组,A组43例患者采用X线胸片联合CT引导下经皮肺穿刺术活检对其进行诊断,B组43例患者采取X线胸片联合电子支气管镜对其进行诊断。比较两组患者经不同诊断方式对各种疾病的检出率,比较两组各种并发症发生情况。结果:电子支气管镜对气道狭窄的检出率明显高于CT引导下经皮肺穿刺术,其他疾病二者无明显差异;行CT引导下经皮肺穿刺术后患者气胸的发生率明显高于电子支气管镜,同时二者均可引起组织损伤。结论:CT引导下经皮肺穿刺术与电子支气管镜对不明原因的肺部病变的诊断有着重要作用,但二者各有利弊,临床工作中应视患者具体病情选择。
Objective:To discuss CT guide percutaneous lung puncture's and electronic bronchoscope's clinical application in unknown causes of lung pathological changes.Methods: 86 Cases of unknown causes of lung pathological changes patients who had been treated at our hospital from september 2009 to september 2012 as clinical study objectives,divided them into group A and group B,each group has 43 cases,adopted X-ray chest radiograph combined with CT guide percutaneous lung puncture to diagnose group A's patients, while adopted X-ray chest radiograph combined with electronic bronchoscope to diagnose group B's patients.Compared two groups' patients'detection rate of disease and compared complication ratios in two groups.Results:Electronic bronchoscope's detection rate of airway stenosis was obviously higher than CT guide percutaneous lung puncture,while with regard to other diseases,there were no evident difference between two groups;Patients who had had CT guide percutaneous lung puncture, happening rate of pneumothorax was obviously higher than patients who had had electronic bronchoscope,meantime,both methods could cause tissue damage.Conclusion:Both CT guide percutaneous lung puncture and electronic bronchoscope have important effect on diagnosis of unknown causes of lung pathological changes,but have their own advantages and disadvantages,in our clinical diaguosis,we should make the choice based on patients' conditions.
