原发性干燥综合征合并非霍奇金淋巴瘤危险因素分析

Risk Factors Analysis of Primary Sjgrens Syndrome Combined with Non-Hodgkin Lymphoma

目的分析原发性干燥综合征(primary Sjgrens syndrome,pSS)合并非霍奇金淋巴瘤(non-Hodgkin lym-phoma,NHL)的相关危险因素。方法对628例pSS患者的性别、临床表现、实验室检查、免疫组织化学(immunohisto-chemistry,IHC)指标、病理学特征进行回顾性队列分析,根据危险因素的暴露与否分为暴露组和非暴露组,通过χ2检验筛选合并NHL的危险因素,经多因素logistic回归分析确定pSS患者合并NHL的独立危险因素。结果 pSS患者NHL的发生率为2.55%,pSS患者中NHL的发生率与性别无关。腮腺肿大、淋巴结肿大、贫血、淋巴细胞减少、粒细胞减少、p53阳性、Bcl-6阳性、PCNA阳性、FS阳性及GC阳性与pSS患者合并NHL有关(χ2值分别为8.917、4.536、5.524、4.604、5.487、52.032、67.280、27.246、7.013、6.092)。腮腺肿大、PCNA阳性、p53阳性、Bcl-6阳性、FS阳性及GC阳性为pSS患者合并NHL的独立危险因素(P<0.05)。结论pSS患者并发NHL的危险因素众多,正确认识这些危险因素对早期诊断和防治具有指导意义。

Objective To analyze the risk factors of primary Sjogrens syndrome （pSS） combined with non-Hodgkin lymphoma （NHL）. Methods The sex, clinical manifestations, laboratory examination, immunohistochemisty, patholog- ical characters of 628 patients with pSS combined with NHI. were analyzed by retrospective cohort study. The patients were divided into exposed group and unexposed group according to the risk factors exposion or not. The risk factors were screened by X^2 analysis, and the independent risk factors were analyzed by multiple factors logistic regression analysis. Re- suits The incidence rate of pSS combined with NHL was 2. 55%, which was not related to sex. The related risk factors of pSS combined with NHL were swelling of parotid gland, lymphadenectasis, anemia, lymphopenia, granulocytopenia, p53 positive, Bcl-6 positive, PCNA positive, FS positive and G2 positive （X^2 = 8. 917,4. 536, 5. 524, 4. 604, 5. 487, 52. 032, 67. 280, 27. 246, 7. 013, 6. 092）. Swelling of parotid gland, PCNA positive, p53 positive, Bcl-6 positive, FS positive and GC positive were the independent risk factors （P〈0. 05）. Conclusion There are multiple risk factors for pSS combined with NHL. The correct recognition of these risk factors helps to direct the early diagnosis, prevention and treatment.
