头孢美唑对大肠埃希菌和肺炎克雷伯菌的抗菌活性观察

摘要 目的了解头孢美唑(CMZ)对大肠埃希菌(ECO)和肺炎克雷伯菌(KPN)体外抗菌活性及其对产超广谱β-内酰胺酶(ESBLs)菌株的作用情况,为临床合理用药提供依据。方法对该院临床各类标本中所分离出的503株ECO和489株KPN进行CMZ耐药性监测,并与其他几种临床常用抗菌药物的耐药性进行比较。结果 ECOESBLs的检出率为62.6%,KPNES-BLs的检出率为39.9%;10种β-内酰胺类药物对ECO和KPNESBLs阴性菌株的耐药率均在10%以下;CMZ对产ESBLs的ECO和KPN的耐药率分别为4.4%和19.0%,这高于亚胺培南(IPM)、美罗培南(MEM)及哌拉西林/他唑巴坦(PIP/TZP),但低于二、三、四代头孢菌素和头孢哌酮/舒巴坦(CFP/SUL)。结论 CMZ对ECO和KPN产ESBLs株具有良好的抗菌活性(80%以上),对于产ESBLs的ECO和KPN引起的感染仍可根据病情选用CMZ。
