彩色多普勒超声对乳腺良恶性肿块诊断的临床价值

Clinical value of color doppler ultrasound in the diagnosis of benign and malignant breast tumor

目的分析彩色多普勒超声对乳腺良恶性肿块诊断的临床价值,以提高对乳腺良恶性肿块的诊断与鉴别诊断率。方法回顾性分析经手术病理证实的162例乳腺肿块的声像图特征,并对良恶性肿块的形态规则、边缘、回声、后方衰减、纵横比、微钙化、血供情况进行对比分析。结果 110例良性肿块彩色多普勒声像图主要表现为:92例形态规则,占83.6%(92/110),101例边缘光滑,占91.8%(101/110),89例内部回声均匀,占80.9%(89/110),99例后方回声正常或增强,占90%(99/110),107例纵横比<1,占97.3%(107/110)。52例恶性肿块组中彩色多普勒声像图主要表现为:44例形态不规则,占84.6%,42例边缘不光滑,占80.8%,回声不均匀共31例,占59.6%,45例后方回声衰减,占86.5%,48例出现微钙化,占92.3%,43例纵横比>1,占82.7%。结论彩色多普勒超声显著提高了乳腺肿块诊断的准确性,在鉴别乳腺良、恶性肿块方面具有重要的诊断价值,值得广泛推广和应用。

Objective To analyze clinical value of color doppler ultrasound in diagnosis of benign and malignant breast tumor,in order to improve the diagnosis of benign and malignant breast tumors and differential diagnosis.Methods A retrospective analysis of 162 pathologically confirmed cases of breast lumps sonographic features of benign and malignant tumors and morphological rules,smooth edges,echo uniform the rear attenuation,aspect ratio,microcalcifications,comparative analysis of the blood supply.Results About 110 cases of benign masses of color doppler sonogram mainly as follow： 92 cases of morphological rules,accounting for 83.6%（92/110）,101 cases of smooth edges,accounting for 91.8%（101/110）,89 cases of internal echo uniform,accounting for 80.9%（89/110）,99 cases of normal or posterior echo enhancement,accounting for 90%（99/110）,the aspect ratio 1 was 107 cases,accounting for 97.3%（107/110）.52 cases of malignant tumor group color doppler sonographic mainly as follows： 44 cases of irregular shape,accounting for 84.6%,42 cases of the edge was not smooth,accounting for 80.8%,a total of 31 cases echo uneven,accounting for 59.6%,45 cases of posterior echo attenuation,86.5%,48 cases of microcalcifications,accounting for 92.3%,43 cases of aspect ratio 1,accounting for 82.7%.Conclusion Color doppler ultrasound significantly improves the accuracy of diagnosis of breast lumps in differentiating benign and evil.Mass has important diagnostic value,should be popularized and applied.
