腔内隔绝术治疗DeBakeyⅢ型夹层动脉瘤临床研究

Clinical research on endovascular graft exclusion in treatment of DeBakey type Ⅲ dissecting aneurysm

目的探讨主动脉腔内隔绝术治疗DeBakeyⅢ型夹层动脉瘤的方法、疗效及安全性。方法对10例DeBakeyⅢ型夹层动脉瘤患者,选择与病变胸主动脉相匹配的移植物,在数字减影血管造影动态监视下,经股动脉导入,当其延降主动脉到达左锁骨下动脉以远时,将移植物从导管内释放,释放后再次行主动脉造影,确定移植物有无扭曲、移位。结果 10例患者成功植入11枚支架,术后7 d、3个月复查胸腹部增强CT和多普勒彩超示:支架无扭曲、移位及内漏。结论腔内隔绝术治疗DeBakeyⅢ型夹层动脉瘤是一种创伤小、恢复快、安全、有效的治疗方法,有良好的临床实用性。

Objective To improve the effects and security in the treatment of aortic dessection.Methods Selected with the lesions thoracic aortic graft match in the 10 patients with DeBakey type Ⅲ dissection patients,importing through the femoral artery in the dynamic monitoring of the digital subtraction angiography,when the extension of the descending aorta reaches the left subclavian artery beyond the graft is released from the catheter,and again after the release of aortic angiography to determine the distortion,shift.Results Stent-grants were successfully performed in all cases,endoleak was performed in all cases.through Thoracic and abdominal enhanced CT,color doppler ultrasonography after 7 days and 3 months.Conclusion Covered stent is the minimal invasive,rapid recovery,safe and effective way to treat Aortic dissection.
