摘要
目的用18F—FDGPETICT显像,评估血清LDH、HBDH、β2-MG对监测非霍奇金淋巴瘤(NHL)结外器官累及的意义。方法回顾分析108例临床确诊NHL患者的18F-FDGPET/CT显像及血清LDH、HBDH、β2-MG测定等临床资料。根据18F—FDGPET/CT显像结果对结外器官累及程度分级,并与血清LDH、HBDH、B2-MG水平进行比较,用ROC曲线对LDH、HBDH、β2-MG的诊断效能进行评价。结臬NHL患者结外器官累及程度越高,血清LDH、HBDH、β2-MG三项生化指标的值越高,各组内差异有统计学意义(P〈0.05);LDH、HBDH和β2-MG诊断NHL结外器官累及的最佳临界值、灵敏度和特异度分别为215UIL、57.9%和83.3%;170U/L、55.6%和75.0%;2.15mg/L、52.3%和74.1%;诊断累及数目〉2,111~2的最佳临界值、灵敏度和特异度分别为271U/L、88.9%和87.2%;28lU/L、85.7%和98.0%;2.38mg/L、90.9%和80.0%。结论血清LDH、HBDH、B,.MG水平可作为NHL患者结外器官是否累及评估的辅助指标。
Objective To evaluate the significance of the lactate dehydrogenase(LDH), eL-hydroxybutyrate dehydrogenase(HBDH), [32-microglobulin( [32-MG) in monitoring non-Hodgkin's lymphoma(NHL) lymph nodes external organs involvement by 18F-FDG PET/CT. Methods 108 patients with NHL underwent 18F-FDG PET/ CT.Among them,LDH,HBDH and [32-MG of 56,56 and 71 patients were measured respectively.The numbers of lymph nodes external organs involvement were divided into three groups(without involving,involving the number 1 - 2 and number 〉 2),according to the results of PEC/CT, and then LDH,HBDH and [32-MG levels were compared.Results The lymph nodes external organs involvement were more,the levels of LDH,HBDH and [3 2-MG were higher(P 〈 0.05).The optimal threshold value,sensitivity and specificity of LDH,HBDH and [3 2-MG in diagnosis of lymph nodes external organs involvement were 215U/L,57.9% and 83.3%, 170U/L,55.6% and 75.0%,2.15mg/L,52.3% and 74. l%,respectively.The optimal threshold value,sensitivity and specificity of LDH,HBDH and [3 2-MG in diagnosis of lymph nodes extemal organs involvement number 〉 2 and 1 - 2 were 271U/L,88.9% and 87.2%,281U/L,85.7% and 98.0%,2.38mg/L,90.9% and 80.0%,respectively.Conclusion LDH,HBDH and β 2-MG can be helpful to monitor external organ involvement in NHL.
