摘要
目的探讨UF-1000i尿沉渣分析仪对脑脊液(CSF)中红细胞(RBC)和白细胞(WBC)计数的准确性,以及细菌(BACT)计数的临床应用价值。方法分别用改良牛鲍计数板和UF-1000i尿沉渣分析仪计数493份CSF样本中RBC和WBC数,并将结果进行比较;用UF-1000i尿沉渣分析仪同时测定的BACT计数结果与Pandy试验蛋白(Pro)定性及细菌(BACT)培养结果进行相关性分析评价。结果 CSF样本中RBC和WBC计数结果在UF-1000i自动化分析与显微镜检验间差异无统计学意义(z=0.831和-1.061,P均>0.05),且对数转换后呈高度相关(r=0.933和0.858,P均=0.000);BACT计数结果与Pro等级呈中度相关(r=0.539,P=0.000),经ROC曲线分析,其曲线下面积(AUC,0.842)明显高于RBC和WBC计数(0.751和0.758,P均<0.01);CSF样本细菌培养阳性19/383例(5.0%),其BACT计数显著高于阴性者(=58.482,P=0.000),以第95百分位数(P95=808.08个/μl)为阈值,细菌培养与BACT计数结果具有较好的一致性(Kappa=0.615,P=0.000)。结论 UF-1000i尿沉渣分析仪可快速、准确完成CSF样本细胞和颗粒成分计数,其BACT计数结果可辅助感染性疾病的诊断。
Objective To explore the accuracy of red blood cell(RBC) and white blood cell(WBC) count in cerebrospinal fluid(CSF) by UF-1000i urinary sediment analyzer,and the clinical application value of bacteria(BACT) count.Methods RBC and WBC of 493 CSF samples were counted by improved neubauer counting chamber and UF-1000i urinary sediment analyzer respectively,and their results were be compared.BACT counts detected simultaneity by UF-1000i urinary sediment analyzer made correlation analysis with the Results of Pandy protein(Pro) test and bacterial culture.Results There was no significant difference(z=0.831 and-1.061,all P0.05) and highly correlation(r=0.933 and 0.858,both P=0.000) by logarithmic transformation between UF-1000i automatic detection and microscopy for RBC and WBC counts of CSF.BACT count had moderate correlation with Pro grades(r=0.539,P=0.000),and its area under the curve(AUC,0.842) was significantly higher than RBC and WBC count(0.751 and 0.758,both P0.01) through ROC curve analysis.19 of 383 cases(5.0 percent) CSF bacteria culture result in the positive.Their BACT count was significantly higher than the negative(=58.482,P=0.000).Setting the threshold with 95 percentile(P95=808.08/μl),bacteria culture and BACT count were with a better consistency(Kappa=0.615,P=0.000).Conclusion UF-1000i urinary sediment analyzer could detect the cell and particle component counts quickly and accurately,and its BACT count possessed an auxiliary effect in infectious diseases diagnosis.
