摘要
目的白细胞介素9及白细胞介素35可能在自身免疫性疾病的发生中发挥重要作用,然而到目前为止尚没有研究证实其与系统性红斑狼疮(systemic lupus erythematosus,SLE)患者的关系。本研究通过检测SLE患者外周血单个核细胞(peripheralblood mononudear cell,PBMC)中CD4+IL-9+T细胞及CD4+EBI3+T细胞百分比,探讨其在SLE患者外周血中的变化及与疾病活动性的关系。方法采用流式细胞仪分别检测28例SLE患者及12名健康体检者外周血中CD4+IL-9+T细胞及CD4+EBI3+T细胞百分比,比较其在正常对照组及SLE患者中的差异及其与部分临床指标的相关性。3组间比较采用方差分析,与临床资料的相关分析采用非参数的Spearmen的相关分析。结果活动期SLE患者外周血中CD4+IL-9+T细胞百分比[(2.11±0.26)%]高于健康对照组[(0.28±0.05)%](P<0.01)和稳定组[(0.86±0.16)%](P<0.01),稳定组高于健康对照组(P<0.05);活动期患者外周血中CD4+EBI3+T细胞百分比[(0.19±0.05)%]低于健康对照组[(1.86±0.67)%](P<0.01)及SLE稳定组[(1.31±0.32)%](P<0.01),稳定组较健康对照组低(P<0.05)。CD4+IL-9+T细胞百分比与抗核抗体(antinuclear antibody,ANA)、抗dsDNA、红细胞沉降率(erythrocyte sedimentation rate,ESR)、24h尿蛋白定量、SLE活动指数(SIE disease activity index,SLEDAI)呈正相关,与C3呈负相关;CD4+EBI3+T细胞百分比与抗dsDNA、ESR、24 h尿蛋白定量、SLEDAI呈负相关,与C3呈正相关。结论 SLE患者外周血CD4+IL-9+T细胞百分比增加,CD4+EBI3+T细胞百分比下降,与疾病活动性相关,提示CD4+IL-9+T细胞及CD4+EBI3+T细胞含量可作为反映SLE活动性的指标。
Objective Interleukin-9 (IL-9) and Interleukin-35 (IL-35) are likely to contribute to the development of autoimmune diseases. Nevertheless, hitherto no studies have shown its role in systemic lupus erythematosus (SEE) in humans. This article aimed to in- vestigate the proportions ofCD4+ EBI3+ 9 T cells and CD4+ EBI3+ T cells in the peripheral blood mononuclear cells of SLE patients and explore the role of these double positive T cells in the pathogenesis of SLE and the relationship of their changes with the activity of the disease. Methods We determined by flow cytometry the percentages of CIM ~ IL-9 ~ T cells and CD4+ EBI3+ T cells in the peripheral blood of 28 SLE patients (13 active and 15 inactive) and 12 healthy controls, and analyzed their differences between the three groups and their correlation with some of the clinical indexes using ANOVA and non-parameterical Spearman's analysis. Results The percentage of CD4+ IL-9+ T cells was significantly increased in the active SLE patients ([ 2.11 -sO. 26 ] % ) as compared with the inactive SLE cases ([ 0. 86 -s O. 16 ] % ) and healthy controls ( [0.28 +0.05]% ) (P 〈0.01), though higher in the inactive SLE than in the control group (P 〈0.05). However, the per- centage of CD4+ EBI3+ T cells was remarkably decreased in the active SLE patients ( [0. 19 .50. 05 ] % ) in comparison with the inactive SLE cases ([ 1.31 ±0. 32] % ) (P 〈0. 01 ) and healthy controls ([ 1.86 + O. 67] % ) (P 〈 0. 01 ), though lower in the inactive SLE than in the control group (P 〈0. 05). The proportion of CD4+ EBI3+ 9 T cells was correlated positively with the titer of ANA and anti-dsDNA, ESR, SLE- DAI and 24-hour urinary protein level, but negatively with the level of C3. Nevertheless, the percentage of CD4+ EBI3+ T cells was correlated CD4+ EBI3+ T ceils can be used as the parameters for the evaluation of SLE activity.
