摘要
目的探讨骨髓及淋巴瘤组织与血液中乙型肝炎病毒的检测是否一致,是否直接破坏骨髓及淋巴组织而导致淋巴瘤。方法将血清HBsAg阳性和HBcAb阳性的恶性淋巴瘤患者分3组:①骨髓及新鲜淋巴结活检标本17例;②淋巴结蜡块标本9例;③10例健康对照者的骨髓标本。应用免疫组化分析每例淋巴结及骨髓标本的乙型肝炎病毒感染情况及感染后对骨髓细胞的影响。结果①11例恶性淋巴瘤患者的骨髓标本免疫组化显示乙型肝炎病毒感染明显,6例乙肝病毒感染不明显;②2例HBV阳性的恶性淋巴瘤患者的新鲜淋巴结标本免疫组化显示乙型肝炎病毒感染明显,15例感染不明显;③9例HBV阳性的恶性淋巴瘤患者的淋巴结蜡块标本免疫组化均未显示乙型肝炎病毒感染;④10例健康对照者的骨髓标本未见乙肝病毒感染。结论乙型肝炎病毒在HBsAg阳性和(或)HBcAb阳性的恶性淋巴瘤患者骨髓中感染明显;在HBsAg阳性和(或)HBcAb阳性的恶性淋巴瘤患者淋巴结组织中无明显感染;尤其是蜡块组织未见表达。
Objective To explore whether HBV in bone marrow and lymphoma is the same as blood, whether HBV leads to leukemia or lymphoma by destroying bone marrow and lymphocyte directly. Methods Malignant lymphoma of clinical serum HBsAg and/or HBcAb positive were devided into 3 groups: (1)the 17 biopsy samples of bone marrow and fresh lymph node, (2)9 samples of lymph node being sealed with wax, (3)10 comparison samples of healthy bone marrow. Immunohistoehemistry was used to analyse each sample of lymph node and bone marrow which was infected with Hepatitis B virus. Results (1)There were 11 bone marrow samples of pernicious lymphoma obviously infected with HBV by immunohistochemistry, and there were 6 samples which were not obviously infected with HBV. (2)There were 2 fresh lymph node samples of pernicious lymphoma obviously infected with HBV by immunohistochemistry, and there were 15 samples which were not obviously infected with HBV. (3)Each of 9 pernieious lymphoma samples which were sealed with wax was not infected with HBV. (4)Ten bone marrow samples of healthy comparison were not infected with HBV. Conclusion It is obviously infected with HBV in bone marrow of pernicious lymphoma patients who are HBsAg and/or HBcAb positive ; it is not obviously infected with HBV in lymph node of pernicious lymphoma patients who are HBsAg and/or HBcAb positive; and it is not infected with HBV especially in lymph node being sealed with wax.
