摘要
目的探讨胰腺、十二指肠损伤的诊断与术式选择。方法对我院1996年1月～2012年12月收治的98例胰腺、十二指肠损伤患者临床资料进行回顾性总结分析。结果本组术前确诊85例(86.7%),术中确诊13例(13.3%),均采取手术治疗。其中治愈88例(89.8%),发生手术并发症5例,死亡5例(5.1%),死亡原因为合并严重颅脑损伤、创伤性湿肺、肺部感染、腹腔感染、感染性休克。结论胰腺、十二指肠损伤的诊断主要根据腹部症状及体征,结合腹腔穿刺、B超、X线、CT检查可确诊。早期诊断、及时和正确的手术方式是提高抢救成功率的关键。
Objective To summarize the diagnosis and treatment of pancreas and duodenum injuries . Methods Clinical data of 98 patients with pancreas and duodenum injuries between 1996 and 2012 were analyzed retrospectively.Results Definitive diagnosis was made in 85 cases preoperatively ,13 cases intra-operatively.All patients received surgical treatment .A total of 88 cases（89.8%） were cured,5 cases（5.10%） died due to severe injury of skull and brain ,pulmonary infection ,infectious shock ,MODS,and hemorrhagic shock .Conclusion Diag-nosis of pancreas and duodenum injuries is mainly based on clinical manifestations .But X-ray,B ultrasound,MR examination can provide important information for its exact diagnosis .Early diagnosis ,timely and appropriate opera-tion are critical to improve the therapeutic effectiveness .
