肺硬化性血管瘤的CT动态增强影像学表现 被引量:10

摘要 目的探讨肺硬化性血管瘤(PSH)的CT动态增强影像学特征。方法回顾分析11例经手术病理证实的PSH的CT表现;所有病例均行多层螺旋CT平扫、动态增强和多平面重组。结果11例均为单发病灶,呈类圆形或椭圆形,边缘光整,仅1个病灶有分叶。病灶最大径为1.2~5.2cm,其中小于3cm者8例,平均2.80em。CT平扫病灶呈等密度,仅1个病灶有细点状钙化。注射对比剂后明显强化,注射至30s时6例呈均匀强化,5例呈不均匀强化,CT值最高达154HU;注射至5min时所有病例呈均匀强化;其中有4例行延迟扫描至15min时,病灶密度CT值仍达90—96HU,平均CT值净增65HU左右。结论PSH具备一般肺内良性肿瘤的特点,CT动态增强影像学特征有助于PSH的诊断。 Objective To evaluate the dynamic contrast-enhanced CT features of pulmonary sclerosing hemangiomas (PSHs). Methods Dynamic contrast-enhanced CT with multi-planar reconstruction of 11 patients with pathologically proved PSHs were studied retrospectively. Results All PSHs were solitary round or oval masses with smooth margins. Only 1 lesion was lobulated. The mean diameter of the tumors was 18 + 5 mm (range 12-52 mm). All lesions were isodense with stippled calcification in 1 case. Enhanced CT scan showed early homogeneous (6) or heterogeneous (5) enhancement 30 seconds after intravenous contrast injection. The maximum CT value of the tumors reached 154 HU. At 5-minutes, all masses were homogeneously enhanced. After 15-minute delay, the CT value increased by an average of 65 HU with 4 masses maintaining 90-96 HU. Conclusions PSHs have characteristics of benign tumors on dynamic contrast-enhanced CT.
