帕金森氏病伴发抑郁的特点及艾司西酞普兰疗效的研究

目的研究帕金森氏病伴发抑郁的特点，观察抗抑郁剂艾司西酞普兰对帕金森氏病伴发抑郁患者康复的影响，为帕金森氏病伴发抑郁的干预提供依据。方法采用临床流行病学调查方法对门诊和住院的198例帕金森氏病患者进行帕金森氏病伴发抑郁及其相关因素的现状调查，运用抑郁自评量表（SDS）、汉密尔顿抑郁量表（HAMD）、日常生活能力量表（ADL）和自制一般情况调查表对帕金森氏病患者进行评定，将帕金森氏病伴发抑郁患者随机分为艾司西酞普兰治疗组和对照组，观察艾司西酞普兰对患者康复的疗效。结果帕金森氏病伴发抑郁的发生率为56．86％，帕金森氏病伴发抑郁的特点为睡眠障碍、绝望、运动阻滞和焦虑等主要表现；帕金森氏病患者HAMD和ADL评分高；口服艾司西酞普兰后治疗组与对照组相比HAMD、HDL减分明显，抗抑郁治疗在缓解抑郁症状的同时，可以改善患者日常生活能力。结论帕金森氏病伴发抑郁发生率较高，以睡眠障碍、绝望、运动阻滞和焦虑等主要表现，抗抑郁剂艾司西酞普兰不仅可以明显改善帕金森氏病伴发抑郁的抑郁症状，还能提高患者的日常生活能力，促进患者的康复。

Objective To explore the features of Parkinson＇s disease Depression （PDD）and observe its influence on rehabilitation of neurological function and the effect of escitalopram therapy and provide the foundation for Parkinson＇s disease Depressionintervention.Methods 198 inpatients and outpatients with stroke were assessed by Self-Rating Depression Scale（SDS）, Hamilton Depression Rating Scale（HAMD）, Activity of Daily Living Scale（ADL） and a self-designed questionnaire. Those PDD patients were randomized into escitalopram therapy group and control group with only regular treatment. Results Among them, 73 cases（36.86%） were identified as suffering from depression; The features of PDD were despair, sleep disorder, movement depress and anxiety. HAMD in patients with Parkinson＇s disease and ADL score is high, After oral escitalopram in treatment group compared with control group the HAMD, HDL decrease points clearly. Escitalopram improved depression, activity of daily living. Conclusion： The frequence of Parkinson＇s disease Depression is high. The features of PDD were despair, sleep disorder, movement depress and anxiety. Escitalopram improved both the depressive symptoms of PDD and activity of daily living of the patients, promote the recovery of patients.
