基础麻醉联合超声引导下IINB在小儿鞘膜积液手术中的应用

摘要 目的观察基础麻醉联合超声引导下髂腹股沟一髂腹下神经阻滞在小儿鞘膜积液手术中的效果。方法40例1.5—5岁择期行鞘膜积液切除术的患儿,ASAI级,随机分为神经阻滞组(NB组)和全麻组(GA组),观察2组一般生命体征、苏醒时间及质量、术后躁动及嗜睡情况、术后出现疼痛时间。结果NB组患儿术中心率、平均动脉压明显较GA组稳定且苏醒快,术后躁动及嗜睡情况发生率均明显低于GA组(P〈0.05),术后镇痛时间长。结论基础麻醉联合超声引导下髂腹股沟一髂腹下神经阻滞可为小儿鞘膜积液手术提供满意的麻醉效果,有利于患儿循环稳定,提高术后苏醒质量,缓解术后疼痛。 objective To observe the effects of combined basal anesthesia and ultrasound guided ilioinguinal/iliohypogastric nerve block during hydrocele of tunica vaginalis surgery in children.Methods Forty children undergoing hydrocele of tunica vaginalis surgery were divided into nerve block group and general anesthesia group at random,Age is 1.5-5 years old,ASA .The HRMAP and SPo2 were continually monitored.The wakefulness time postoperative restlessness lethargy and the time of pain appearing were recorded.Results The HR and MAP in NB group were more stable than those in GA group and the recovery period was shorter.Postoperative restlessness and lethargy were lower in NB group than those in GA group.NB has very good analgesic properties.Conclusion Combined basal anesthesia and ultrasound guided ilioinguinal/iliohypogastric nerve block during hydrocele of tunica vaginalis surgery in children is effective and safe.lt will help to stabilize circulation,improve the quality of recovery period and postoperative analgesia.
