原发性高血压左心结构改变患者的PTFv1改变与心律失常的关系

Relationship between change of PTFv1 on ECG and arrhythmias in change of primary hypertensive left cardiac structure

目的 :探讨原发性高血压左心结构改变患者的心电图 V1 导联 P波终末电势 (PTFv1 )改变与心律失常的关系。方法 :检测 1 2 8例 PTFv 1负值增大的原发性高血压左心结构改变的患者的动态心电图 ,并以 61例 PTFv 1正常者为对照。结果 :PTFv 1负值增大组 :(1 )单纯左心房内径增大者房性心律失常 (AA)检出率显著高于正常组 (78.4% vs 47.8% P<0 .0 5) ;(2 )单纯左心室肥厚者室性心律失常 (VA)检出率显著高于正常组 (68.6% vs39.8% P<0 .0 5) ;(3)左心房内径增大并左心室肥厚者 AA检出率显著高于正常组 (71 .4%vs 41 .2 % P<0 .0 5)。结论 :原发性高血压左心结构改变患者的心电图 PTFv 1值可能成为心律失常的辅助预测指标。

Objective:To study relationship between change of terminal force of P wave in lead v1(PTFv 1) on electrocardiogram(ECG) and arrhythmias in change of primary hypertensive left cardiac structure Methods:24 hours dynamic electrocardiogram monitoring were recorded in 128 patients with abnormal PTFv1(≤-0 04 mm·s)and 61 patients with normal PTFv 1(≥ 0 02 mm·s) in change of primary hypertensive left cardiac structure Results:(1)It was higher that incidence of atria arrhythmias (AA) detection in the left atrial enlargement (LAE) with abnormal PTFv 1 than that with normal PTFv 1 (78 4% vs 47 8% P <0 05) (2)It was higher that incidence of ventricular arrhythmias (VA) detection in the left ventricular hypertrophy (LVH) with abnormal PTFv 1 than that with normal PTFv 1 (68 6% vs 39 8% P <0 05) (3)It was higher that incidence of AA detection in the LAE and LVH with abnormal PTFv 1 than that with normal PTFv 1(71 4 % vs 41 2% P <0 05) Conclusion:PTFv1 on ECG may be one of standard for arrhythmias detection in change of primary hypertensive left cardiac structure
