不同剂量垂体后叶素对腹腔镜子宫肌瘤剔除术中循环的影响

Comparison on the influence of different doses pituitrin for circulatory system of parturient underwent laparoscopes myomectomy

目的探讨不同剂量垂体后叶素对量对腹腔镜子宫肌瘤剔除术中循环系统的影响。方法将100例腹腔镜子宫肌瘤剔除术患者随机分为A组和B组,A组患者给予垂体后叶素6 U在子宫肌层进行注射,B组患者给予垂体后叶素12 U在子宫肌层进行注射,观察两组患者手术情况及不同时间段心率(HR)、血压(BP)、血氧饱和度(SpO2)变化情况。结果两组患者手术时间,术中出血量、血红蛋白下降水平、术后排气时间、术后住院时间差异均无统计学意义(均P>0.05),两组患者HR和DBP在注射后3、4和5 min均有明显升高,HR在8 min和10 min明显下降,与注射后1 min水平比较,差异均有统计学意义(P<0.05),并且A组HR在5 min水平明显低于B组,差异有统计学意义(P<0.05),两组患者SBP在注射后3 min开始升高,且B组患者升高水平明显高于A组,差异有统计学意义(P<0.05)。结论腹腔镜子宫肌瘤剔除术应用小剂量垂体后叶素能有效减少术中出血量,并避免剧烈的循环波动,值得临床推广应用。

Objective To comparison on the influence of different doses pituitrin for circulatory system of parturient underwent laparoscopes myomectomy.Methods 100 cases were randomly divided into group A and group B,group A given myometrium injection with pituitrin 6 U,B group given myometrium injection with pituitrin 12 U,observing two groups changes of operation conditions and different times the heart rate（HR）,blood pressure（BP）,blood oxygen saturation（SpO 2）.Results In the two groups of patients with operation time,intraoperative blood loss,hemoglobin level,postoperative exhaust time,postoperative hospital stay were no significantly statistical difference（P 0.05）,two groups of patients with HR and DBP in 3 min after injection,4 min,5 min has the obvious rise,HR in 8 min and 10 min significantly decreased,it was higher than 1 min after the injection level,differences were significant statistical significance（P 0.05）,Group A HR 5min level was significantly lower than in group B,the difference was statistically significant（P 0.05）,two groups of patients with SBP in 3 min after injection began to heighten,and the higher level is significantly higher than group A,group B had obvious statistical significance difference（P 0.05）.Conclusions Laparoscopic uterine myoma eliminating technique application of small dosage pituitrin can effectively reduce the intraoperative blood loss,and to avoid severe cycle fluctuations,worthy of clinical popularization and application.
