腹腔镜直肠癌根治术术前个体化护理策略的应用及评价

Application and evaluation of laparoscopic radical resection of rectal carcinoma preoperative individualized nursing strategies

目的探讨腹腔镜直肠癌根治术术前个体化护理策略应用的可行性及效果。方法将140例拟行腹腔镜直肠癌根治术手术患者随机分为观察组和对照组各70例，观察组应用术前个体化护理策略，对照组采用常规护理方法。比较两组术后自觉症状、生理功能、日常生活、社会活动、精神情绪状态。结果在术后12个月的问卷调查中，各项指标统计分值显示：CIQLI指数对照组平均总分89．13分，而观察组平均总分高达112．19分，差异有统计学意义（P〈0．01）。说明观察组经个体化干预后，生存质量的各项指标高于对照组，主要表现为观察组在精神情绪状态、日常生活和社会活动方面明显优于对照组。结论术前个体化护理组策略在腹腔镜直肠癌根治术手术前能树立病人战胜疾病的信心，明显减轻患者术前的焦虑、痛苦，加速术后康复进程，提高患者的生存质量。在腹腔镜直肠癌根治术治疗中是安全可行的。

Objective To observe the clinical effects and feasibility of applying individual nursing strategies before laparoscopic radical resection of rectal carcinoma surgery. Methods A total of 140 cases of rectal cancer patients undergoing laparoscopic surgery were randomly divided into 2 groups, 70 patients were treated with effective individualized nursing in the observation group, while the control group 70 cases were not. Their postoperative symptoms, physical function, daily life, social activities, mental or emotional state were com- pared. Results All patients completed the experiment. Twelve months after the survey, GIQLI index of the control group scored 89. 13 points meanly, while the observation group, the average score was as high as 112. 19 points, the difference of which was statistically signifi- cant with P 〈 O. 01, which means after the intervention by the individual care, the patients got a higher life quality, mainly, the observation group were better in the mental or emotional condition, daily life and social activities than the control group. Conclusions Preoperative individualized nursing group strategies in laparoscopic rectal cancer patients are advantageous for patients to establish the confidence to overcome the disease, significantly reduce preoperative anxiety, pain, accelerate postoperative rehabilitation process, improve patients＇ quality of life, which is safe and feasible in the laparoscopic radical resection of rectal carcinoma surgery.
