摘要
目的探讨尿液有形成分分析对尿路感染的临床诊断价值,以便通过尿液有形成分分析达到快速诊断临床尿路感染的目的。方法用UF-1000i尿有形成分分析仪检测625份临床尿液标本的白细胞、细菌及酵母样真菌计数,同时对该尿标本进行细菌培养和鉴定,以患者临床诊断作为"金标准",评估UF-1000i尿有形成分分析仪的检测对尿路感染诊断价值。结果尿液培养阳性162例,阳性率为25.92%,白细胞和细菌计数阳性210例,阳性率为33.60%,有形成分检出率高于培养法检出率(χ2=31.41,P<0.01;χ2=138.51,P<0.01),联合白细胞及细菌计数检测的敏感度、阴性预测值较培养法高。结论尿液有形成分分析对尿路感染的早期筛查有重要作用,对尿细菌培养为阴性患者的明确诊断也具有重要价值。
OBJECTIVE To investigate the application value of analysis of urine formed elements in diagnosis of urinary tract infections so as to achieve the purpose of rapid diagnostic clinical urinary tract infections by analyzing the urine formed elements. METHODS A total of 625 clinical urine specimens were analyzed by UF-1000i in parallel through detection of leukocyte, bacteria and yeast-like fungus, while these samples were cultured for bacterial identification. Then the value of UF-1000i in diagnosis of urinary tract infections was assessed with the clinical diagnosis as the gold standard. RESULTS There were 162 cases with the urine cultured positive, and the positive rate was 25.92 %, there were 210 cases with both leukocyte and bacteria counts tested positive, and the positive rate 33.60%, the positive rate of UF-1000i was higher than that of the culture method(x2 = 31.41, P〈0.01 %;x2= 138.51 ,P〈0. 01). As compared with culture, both the sensitivity and the negative predicative value of the joint test of leukocyte and bacterial counts were higher. CONCLUSION Urinary formed elements analysis plays a very important role in early screening of urinary tract infections, especially for the patients with the urine cultured negative.
