摘要
目的观察新诊断中青年2型糖尿病患者合并原发性高血压24 h动态血压(ABPM)的变化及其临床特点。方法对106例新诊断的中青年2型糖尿病合并原发性高血压患者(A组)行ABPM检查,分析其可能的变化及其临床特点,另设106例单纯的中青年原发性高血压患者(B组)作为对照。结果 A组白天平均收缩压(平均为162±18 mmHg)高于B组(平均为148±12 mmHg)(P<0.05);A组白天和夜间SBP分别(42±26)%和(74±38)%,白天和夜间DBP负荷分别为(10±12)%和(12±14)%,B组白天和夜间SBP分别(28±25)%和(40±34)%,白天和夜间DBP负荷分别为(6±9)%和(9±10)%,A组白天和夜间SBP及DBP负荷明显高于B组(P<0.01);A组SBP及DBP负荷昼夜差值明显低于B组(P<0.01);A组异常血压形态(非杓形)及节律消失者为62%和89%,而B组为36%和57%,A组血压形态非杓形及节律消失者明显高于B组(P<0.01)。结论新诊断中青年2型糖尿病患者合并原发性高血压者收缩压较高,血压负荷值增高,而ABPM昼夜节律消失和异常血压形态比例明显增高。
Objective To observe the changes and its clinic characterestics of 24h ambulatory blood pressure monitoring(ABPM) in new diagnostic young and middle-aged diabetic patients with primary hypertension.Methods One hundred and six new diagnostic young and middle-aged diabetic patients with primary hypertension(group A) were examined using ABPM,meanwhile its possible changes and clinical characteristics were analyzed.One hundred and six young and middle-aged patients with primary hypertension(group B) were as control group.Results The daytime mean systolic blood pressure(SBP) in group A(162 ± 18 mmHg) was higher than that in group B(148 ± 12mmHg,P0.05).The daytime and night SBP in group A were 42%±26% and 74%±38%,while in group B were(28±25)% and(40±34)%.The daytime and night DBP in group A were 10%±12% and 12%±14%,while in group B were 6%±9% and 9%±10%.The daytime and night SBP and DBP load in group A were obviously higher than those in group B,while daytime and night SBP and DBP load differences in group A were obviously lower than those in group B(P0.01).The percent of abnormal blood pressure form(non-scoop shape) and rhythm disappear in group A were 62% and 89%,which were significantly higher than in group B(36% and 57%,P0.01).Conclusion The new diagnostic young and middle-aged diabetic patients with primary hypertension have higher systolic blood pressure and blood pressure load value,meanwhile,the probability of ABPM diurnal rhythm disappear and abnormal blood pressure form are increased obviously.
