摘要
目的探讨2型糖尿病患者与健康人房水中血管内皮生长因子及视网膜色素上皮源性因子水平是否存在差异。方法糖尿病患者组39例39只眼,存在糖尿病性视网膜病变(diabetic retinopathy,DR)患者18例18只眼,无DR患者21例21只眼,老年性白内障患者20例20只眼作为对照组。用酶联免疫吸附法检测房水中VEGF及PEDF的水平。结果在2型糖尿病组中存在DR及无DR患者房水中VEGF浓度分别为(1 241.39±274.71)pg/ml及(1 183.31±231.12)pg/ml,与对照组[(951.47±280.39)pg/m1]相比,差异有统计学意义(P=0.004,P=0.013)。对照组、DR组及无DR组患者PEDF浓度分别为(40.11±15.87)ng/ml、(50.13±46.73)ng/ml、(34.68±4.77)ng/ml,与对照组相比,无DR患者房水中PEDF浓度明显降低(P=0.042)。对照组、DR组及无DR组患者房水中VEGF/PEDF分别为26.15±11.92、32.41±12.11、34.58±7.97,与对照组相比,其余两组患者房水中VEGF/PEDF明显升高(P=0.038,P=0.002)。结论糖尿病患者房水中VEGF水平及VEGF/PEDF升高,提示2型糖尿病患者房水中VEGF与PEDF的失衡可能与糖尿病性视网膜病变的发生、发展有关。
Objective To determine the aqueous levels of vascular endothelial growth factor (VEGF) and pigment epithelium-derived factor (PEDF) in type 2 diabetic patients with or without diabetic retinopathy. Methods Aqueous humor samples were collected from 39 eyes of 39 type 2 diabetes patients, including 21 eyes without diabetic retinopathy and 18 eyes with diabetic retinopathy. Twenty aqueous samples from 20 cataract patients without other ocular or systemic diseases served as controls. The VEGF and PEDF levels in aqueous humor were measured by enzyme-linked immunosorbent assay (ELISA). Results The mean levels of VEGF in eyes with diabetic retinopathy [ (1 241.39± 274.71 ) pg/ml, P = 0. 004 ]and without diabetic retinopathy[(1 183.31 ± 231. 12) pg/ml, P= 0. 013 ] were significantly higher than that in controls (951.47 ± 280. 39 ) pg/ml 1. The PEDF level in eyes without diabetic retinopathy I ( 34.68 ± 4.77 ) ng/ml, P = 0. 042 ~ was significantly lower than that in controls[ (40.11± 15.87 ) ng/ml ]; while there was no significant difference in PEDF levels between diabetic retinopathy patients (50.13 ±46.73) ng/ml] and controls (P=0. 121 ). The ratio of VEGF/PEDF in eyes with diabetic retinopathy (32.41 ± 12.11, P = 0. 038 ) and without diabetic retinopathy ( 34.58 ± 7.97, P = 0. 002 ) were significantly higher than that in eyes with cataract alone ( 26. 15 ± 11. 92 ). Conclusion The aqueous level of VEGF and the ratio of VEGF/PEDF are up-regulated in type 2 patients, which may be related to the development and progression of diabetic retinopathy.
