中枢性原始神经外胚层肿瘤的CT、MRI表现和病理分析

CT,MRI Features and Pathological Analysis of CNS PNET

目的探讨中枢性原始神经外胚层肿瘤(cPNET)的CT、MRI表现及其病理联系。方法回顾性分析11例颅内cPNET患者的CT、MRI资料,并结合文献复习及病理分析。结果 cPNET好发于儿童和青少年,肿瘤体积多较大,可发生于脑任何部位,呈类圆形、分叶状或梭形。肿瘤大部分呈实性或偏实性,可伴有坏死囊变、出血或钙化。肿瘤实性部分CT上多呈稍高密度,MRI上多呈稍长T1、稍长T2信号,呈均匀或不均匀明显强化。结论 cPNET在CT、MRI上有一定的特征性表现,但影像诊断仍存在一定困难,最终确诊依赖于病理。

Objective To investigate the CT, MRI features and pathology relationship of CNS primitive neuroectoder- mal tumors. Methods The CT and MRI findings of 11 cases with CNS primitive neuroectodermal tumors proved by pathology were analyzed retrospectively and combined with literature and pathology. Results cPNET often occurred in children and young, which had large tumor volume and could occurred at any position of brain with round shape, sublobe or fusifoim. Tumor of 10 cases showed solid lesions or predominantly solid lesions, which accompanied with necrosis, cystic degeneration, hemorrhage or calcification. Solid components of all tumors showed hyperdensity on CT scan and slightly hypointensity signal on T1 W, slightly hyperintensity signal on T2W, which demonstrated obvious homogeneous or inhomogeneous enhancement. Conclusion The CT, MR imaging of cPNET have some characteristics, however, definite diagnosis depends on pathology.
