摘要
目的调查糖尿病与消化性溃疡患者幽门螺杆菌感染差异,为根除幽门螺杆菌提供依据。方法选取官渡区人民医院就诊糖尿病患者及消化性溃疡患者各300例,选择此期间就诊的年龄、性别、居住环境与其相匹配的健康体检人群200例。所有患者均行14C呼气试验行H.pylori感染检测,计算3组H.pylori感染率及血清H.pylori抗体IgG的滴度的高低。详细询问检查糖尿病症状与并发症、记录糖尿病病程。结果糖尿病组、消化性溃疡组和正常人组在60岁以下,H.pylori感染率有逐年上升的趋势。糖尿病组和消化性溃疡组较正常组的H.pylori感染率高,消化性溃疡组较糖尿病组的H.pylori感染率高。糖尿病组H.pylori感染率随着糖尿病病程的进展呈上升趋势。合并H.pylori感染的糖尿病患者和消化性溃疡患者与合并H.pylori感染的正常人比较,H.pylori感染滴度差异有统计学意义(P<0.05)。H.pylori感染滴度在1 000以上时,合并H.pylori感染的糖尿病患者与合并H.pylori感染的消化性溃疡患者比较,H.pylori感染滴度差异有统计学意义(P<0.05)。合并H.pylori感染的糖尿病患者较无H.pylori感染的糖尿病患者的并发症发生率明显增高,差异有统计学意义(P<0.05)。结论糖尿病患者的H.pylori感染风险、感染滴度较正常人增高,并随病程的进展有上升趋势,H.pylori感染与糖尿病各种并发症关系密切。
Objective To investigate the difference of Helicobacter pylori (H. pylori) infection in diabetics and peptic ulcer patients and provide the basis for eradication of Helicobacter pylori. Methods A parallel-controlled study was conducted among 300 diabetics, 300 peptic ulcer patients and 200 healthy people in Guandu District People' s Hospital. All patients underwent 14 C breath test for the detection of H. pylori infection, H. pylori infection rate and the level of serum anti-H, pylori IgG antibody titer were calculated in three groups. The symptoms and complications of diabetes, dura- tion of diabetes were recorded. Results Under the age of 60, H. pylori infection rate in diabetic group, peptic ulcer group and normal group had an increasing trend. Compared with H. pylori infection healthy people, H. pylori infection titers were significantly different in combined H. pylori infection diabetic patients and combined H. pylori infection peptic ulcer patients. In H. pylori infection titers above 1 000, Compared with combined H. pylori irffection diabetes patients, H. pylori infection titers were significantly different in combined H. pylori infection peptic ulcer patients. Compared with diabetes patients without H. pylori infection, incidence of complications was significantly higher in combined H. pylori in- fection diabetes patients. Conclusion Risk of H. pylori infection is higher in diabetic patients than healthy people. H. pylori infection and complications of diabetes are closely related.
