小剂量瑞芬太尼、地佐辛在全麻拔管期的应用

Small dose of remifentanil cardiovascular response during decannulation period of general anesthesia

目的研究小剂量瑞芬太尼、地佐辛对全麻拔管期心血管反应的预防作用。方法选择60名全麻手术患者,ASA I-Ⅱ级,随机均分为小剂量瑞芬太尼组和地佐辛组,各30例。比较2组用药前、拔管时及拔管后1、5、10、20 min的心率(HR)、中间动脉压(MBP)的变化。结果 2组拔管时及拔管后1、5 min MBP、HR与用药前比较均无显著升高(P>0.05),小剂量瑞芬太尼组10、20min MBP、HR较用药前变化明显(P<0.05)。地佐辛组10、20 min MBP、HR较用药前无明显变化(P>0.05)。结论小剂量瑞芬太尼、地佐辛能有效预防全麻拔管期的心血管反应,后期地佐辛作用优于小剂量瑞芬太尼。

Objective to study the small dose of remifentanil, dezocine prevent cardiovascular reaction during the decannulation period of general anesthesia. Methods 60 patients with general anesthesia surgery, ASA grade I or Ⅱ , randomly were divided into low - dose remifen- tanil group and dezocine group,30 cases in each group. Compare the two groups before medication, extubation and after extubation 1 min, 5 min,lO min,20 rain among HR,MBP changes. Results MBP,HR when the tube drawing and after extubation 1 min,5 min had no significant increase than those before medication in two groups （P 〉 0.05 ）, MBP, HR has change significantly after extubationl0 min ,20 min in small dose of remifentanil group（P 〈0.05）. MBP,HR has no significant change after 10 min,20 min in Dezocine group （P 〉 0.05 ）. Conclusion small dose of remifentanil,dezocine can effectively prevent the cardiovascular response during decannulation period of general anesthesia, dezocine effect is better than that of small dose of remifentanil in the later time.
