抗菌药物临床应用专项整治深入开展前后围手术期预防用药的成效分析

Effect Analysis of Perioperative Prophylactic Antibiotics before and after Strict Administration of Clinical Antibiotics Application

目的了解抗菌药物临床应用专项整治深入开展前后本院围手术期抗菌药物预防性应用的成效情况。方法采取回顾性调查方法,随机抽查深入开展前2011年与深入开展后2012年Ⅰ、Ⅱ类切口手术分别247例与339例,分析2个时间段围手术期抗菌药物预防性使用情况,比较手术切口感染的发生率。结果抗菌药物临床应用专项整治深入开展前后,Ⅰ类切口手术抗菌药物使用率进一步降低,分别为65.4%与24.1%;术前给药时机合理率,由74.9%提升至92.5%;术后用药疗程在24 h内的2011年为27.8%,2012年为69.8%;而手术切口感染率变化不大。结论抗菌药物临床应用专项整治深入开展,在围手术期抗菌药物预防性使用方面取得一定的成效,但仍需进一步提高合理用药水平。

Objective To analyze the effects of perioperative prophylactic antibiotics before and after the strict administration of clinical antibiotics application.Methods A retrospective investigation method was used to analysis 243 cases of Type I and II incision operations in 2011 before the strict administration of clinical antibiotics application,and 339 cases in 2012 after the strict administration.The situation of perioperative prophylactic antibiotics was analyzed during these 2 different periods.And the incidence rates of surgical site infection were compared too.Results The utilization rate of perioperative prophylactic antibiotics in Type I incision operations decreased from 65.4% to 24.1% after the strict administration of clinical antibiotics application.The rational ratio of administering opportunity before the operation increased from 74.9% to 92.5%.The postoperative courses of antibiotics which less than 24 h was 27.8% in 2011 increased to 69.8% in 2012.The incidence rates of surgical site infection were similar in 2011 and 2012.Conclusion With the further development of strict administration of clinical antibiotics application,there are some achievements obtained and the rational antibiotics application still need to be further improved.
