Salvage operation for the complete disconnection of choledochus liver artery and portal vein in subtotal gastrectomy

目的 :探讨胃大部切除术中胆总管、肝动脉、门静脉完全离断的预防措施及抢救方法 .方法 :分析 4例胃大部切除术中胆总管、肝动脉、门静脉完全离断损伤的原因以及抢救过程中处理胆总管、肝动脉、门静脉的方法和顺序 .结果 :按照门静脉、肝动脉、胆总管的先后顺序完善吻合并引流 ,抢救取得成功 .结论 :首先恢复门静脉的连续性 。

Objective:[WT5BZ][ST5BZ]To establish the prevention and salvage methods for the complete disconnection of choledochus,liver artery and portal vein and treatment in subtotal gastrectomy.[WT5HZ]Methods:[WT5BZ]Four cases were analyzed to explore the reason and treatment for the complete disconnection.[WT5HZ]Results:[WT5BZ]We concluded that the salvage operation was successful when the following connection and drainage order was followed:portal vein→liver artery→cholechus.[WT5HZ]Conclusions:[WT5BZ]In the salvage operations,it is most important to restone the succession of portal vein first,so that the liver warm ischemia time can be decreased to the safe range.
