小关节内侧缘入路盘内胶原酶溶解联合射频热凝治疗二区C域Ⅲ层腰椎间盘突出症的临床研究 被引量:4

摘要 目的:观察小关节内侧缘入路盘内胶原酶溶解联合射频热凝治疗二区C域Ⅲ层腰椎间盘突出症的临床疗效。方法:336例二区C域Ⅲ层腰椎间盘突出症患者使用随机数字表进行完全随机分组的方法分为A、B二组,每组168例。A组采用小关节内侧缘入路盘内胶原酶溶解联合射频热凝术治疗,B组采用小关节内侧缘入路盘内胶原酶溶解术治疗。采用MacNab疗效评定标准评估治疗效果。观察二组患者术后2周、3个月的治疗效果。结果:术后2周和3个月,A组治疗优良率高于B组。结论:小关节内侧缘入路盘内胶原酶溶解联合射频热凝治疗二区C域Ⅲ层腰椎间盘突出症疗效优于单独应用椎板间孔入路盘内胶原酶溶解术。 Objective: To observe the therapeutic effect of collagenase dissolved into the disc through the route of inner margin of small joint combined with radiofrequency thermocoagulation on lumbar disc herniation at second zone area C level III. Methods: A total of 336 cases with lumbar disc herniation at second zone C area III level were randomly divided into groups A and B with 168 cases in each group. The technique of collagenase dissolved into the disc through the route of inner margin of small joint combined with the therapy of radiofrequency thermocoagulation was applied in group A. The technique of collagenase dissolved into the disc through the route of inner margin of small joint was applied in group B. The MacNab criteria were used to evaluate the treatment efficacy. The clinical efficacy in patients was observed at the time points of 2 weeks and 3 months postoperatively. Results: The rates of excellent effect were higher in group A than group B at 2 weeks and 3 months after operation respectively. Conclusion: The efficacy of the technique of collagenase dissolved into the disc through the route of inner margin of small joint combined with radiofrequency thermocoagulation to treat lumbar disc herniation at second zone area C level III is superior to the treatment of collagenase dissolved into the disc through the route of inner margin of small joint alone.
