广西地区降低TSH阳性切值对新生儿疾病筛查的影响

目的探讨TSH阳性切值降低对新生儿疾病筛查是否有影响。方法降低卫生部TSH阳性切值对2011-2012年收到共460 295例广西地区新生儿干血片进行疾病筛查。结果按卫生部规定的TSH阳性切值10μIU/ml,筛查出可疑阳性2727例(其中被召回2315例),确诊甲低210例,高TSH血症238例;经统计将TSH阳性切值降为8μIU/ml后筛查出可疑阳性5799例,增加3072例(其中被召回4878例,新增召回2563例),确诊甲低229例(新增19例甲低),高TSH血症300例(新增62例高TSH血症)。结论在提高干血片质量和实验室质控在控减少假阳性前提下,降低TSH阳性切值对广西地区新生儿疾病筛查产生重要影响。

Objective： To investigate TSH positive cut value reduced to the newborn screening for the disease have an impact. Methods ： To reduce the ministry of health TSH positive cut value on 2011 - 2012 received a total of 460 295 cases of the region of GuangXi neonatal dried blood spots for disease screening. Results： According to the provisions of the ministry of health TSH positive cut value 10μIU/mt, screening out of 2727 cases of suspected positive （including recalled 2315 ）, and confirmed CH 210 cases, 238 cases of High TSH acidosis; according to the statistics TSH positive cut value reduced to 8μIU/ml screen out 5799 cases of suspected positive, an increase of 3072 cases （recalled 4878 cases, increase of 2563 cases）, confirm CH 229 cases （19 cases with a new）, high TSH acidosis 300 cases （62 new cases of high TSH acidosis）. Conclusion： Conditions of improving dry blood spots quality and the Laboratory quality control to reduce false positives, reduce the TSH positive cut value on the region of GuangXi newborn screening disease have an important impact.
