腹腔镜卵巢囊肿剔除术中电凝与缝合止血对卵巢功能影响的比较

Effect Comparison of Electric Coagulation and Suture Hemostasis on Ovarian Function in Laparoscopic Ovarian Cyst Resection

目的:比较腹腔镜卵巢囊肿剔除术中电凝止血与缝合止血对卵巢功能的影响。方法:90例行腹腔镜卵巢囊肿剔除术患者按信封密封法分为电凝组和缝合组,每组45例。电凝组采用双极电凝对卵巢创面止血,缝合组采用3-0可吸收线缝合卵巢对剥离面止血。比较两组手术情况、术前后激素水平变化及排卵、月经情况。结果:两组手术时间和术中出血量无显著差异(P>0.05)。电凝组术后1个月和术后3个月的E2显著低于缝合组(P<0.05),FSH显著高于缝合组(P<0.05),LH与缝合组无显著差异(P>0.05)。电凝组排卵异常、经期延长和经量过少均显著多于缝合组(P<0.05)。结论:电凝止血较缝合止血对卵巢组织损伤大。

Objective： To compare the effect of electric coagulation and suture hemostasis on ovarian function in laparoscopic ovarian cyst resection.Methods：According to sealed envelope method,90 cases of patients elective for laparoscopic ovarian cyst resection were divided into coagulation group and the suture group with 45 cases in each group.The coagulation group used the bipolar coagulation to stop the bleeding of ovarian wound,the suture group sutured ovarian with 3-0 absorbable suture to stop bleeding of dissection surface.The surgery information,hormone levels changes before and after surgery and ovulation,menstruation were compared of two groups.Results： The operative time and bleeding volume of two groups compared,the difference was not statistically significant（P＆gt;0.05）.The E2in coagulation group was significantly lower than that of suture group at 1 month and 3 months after surgery（P＆lt;0.05）,while the FSH was significantly higher than the suture group（P＆lt;0.05）,LH has no significant difference compared with suture group（P＆gt;0.05）.The abnormal ovulation,menstrual extended and menstrual volume less of coagulation group were significantly more than the suture group,the difference was statistically significant（P＆lt;0.05）.Conclusion： Compared with suture hemostasis,the electric coagulation hemostasis has more injury on ovarian tissue.
