几种不同内固治疗成人肱骨远端骨折的临床观察

Clinical Observation of Several Different Internal Fixation of Distal Humerus Fracture Treatment of Adult

目的探讨几种不同内固治疗成人肱骨远端骨折的临床效果。方法选取1991年1月至2012年1月我院收治的成人肱骨远端骨折患者160例进行分析,按照内固定方式分成A、B、C、D四组,每组40例,A组采用克氏针内外髁八字内固定治疗,B组采用重建钢板内固定治疗,C组采用解剖丫型钢板内固定,D组采用两块解剖锁定钢板内固定治疗,观察四组治疗效果。结果 D组手术时间、术中出血量、骨折愈合时间明显少于A、B、C三组(P<0.05);D组肘关节功能评分明显高于A、B、C三组(P<0.05);四组均无严重并发症发生。结论两块解剖锁定钢板内固定治疗成人肱骨远端骨折手术时间短,术中出血量少,骨折可以获得坚强的内固定,能够加快骨折愈合,改善患者肘关节功能,是一种安全有效的治疗方法,值得临床推广。

Objective To explore the clinical effect of several different solid treatment of adult distal humerus fractures. Methods 160 patients with distal humerus in adult analyzed from 1991 January to 2012 January in our hospital were divided into four groups A, B, C, D, in accordance with the internal fixation, 40 cases in each group, group A with Kirschner wire fixation, group B with reconstruction plate fixation,group C with anatomical plate fixation, group D with two anatomic locking plate fixation, to observe the treatment effect of the four groups. Results Group D in operation time, amount of bleeding, fracture healing time was less than that of the group A, B, C（P〈0.05）; Group D elbow function score was significantly higher than that of the group A, B, C（P〈0.05）; no serious complications occurred in four groups. Conclusion Two anatomic locking plate fixation operation time short plate for the treatment of adult distal humerus, less bleeding, can accelerate fracture healing, improve the elbow joint function of patients, is a safe and effective treatment method, is worth of the clinical promotion.
