心理护理在米索前列醇与取环钳联合用于绝经后取器的临床观察

Clinical observation of psychological nursing in combined treatment of misoprostol and clamp in removing intrauterine device for postmenopausal women

目的观察在米索前列醇与取环钳联合治疗绝经后取器中应用心理护理的效果。方法选取浙江省丽水市妇幼保健院2011年6月～2012年6月要求宫内取器的绝经期女性120例,按照随机数字法分为对照组和观察组,两组女性均在术前给予米索前列醇配合取环钳取宫内节育器。观察组另给予术前、术中以及术后心理护理措施,对照组不给予心理护理,观察两组取环效果、宫颈软化情况、疼痛情况、取环时间、术中出血量、护理满意度、抑郁与焦虑评分。结果观察组取环顺利率、困难率、宫颈软化优良率、疼痛程度、术中出血量和手术时间以及护理满意度评分与对照组比较,差异均有统计学意义(P<0.05)。治疗前两组患者的SDS评分和SAS评分比较,差异无统计学意义(P>0.05);治疗后两组的SDS和SAS评分比较,差异有统计学意义(P<0.05)。结论绝经期女性取器术中实施心理护理能够有效提高宫内节育器取出成功率,减轻女性痛苦,改善抑郁与焦虑状况,值得临床推广应用。

Objective To observe the effect of psychological nursing in combined treatment of misoprostol and clamp in removing intrauterine device for postmenopausal women. Methods A total of 120 cases of postmenopansal women who demanded to remove the intrauterine device, and admitted to Maternal and Child Care Service Centre of Lishui City in Zhejiang Province from June 2011 to June 2012, were randomly divided into control group and observation group, all the patients were treated with misoprostol and clamp to remove intrauterine device before the operation, and the patients in observation group were treated with preoperative, intranperative and postoperative psychological nurs- ing, while the patients in control group were treated with no psychological nursing, then observed the effect of removing intrauterine device, soften of cervix, pain, time of removing intrauterine device, intraoperative blood loss, nursing satisfaction, depression and anxiety score. Results The successful rate and difficult rate of removing intrauterine device, the excellent and good rate of soften of cervix, degree of pain, intraoperative blood loss, time of operation, and the nursing satisfaction score of the observation group was significantly different than that of the control group （P〈0.05）. There was no significant difference between the two groups in SDS scores and SAS scores before the treatment （P〉 0.05）, and there was significant difference after the treatment （P〈0.05）. Conclusion Psychological nursing can effectively improve the success rate in removing intrauterine device for postmenopausal women, relieve the pain and im- prove the depression and anxiety, so it＇ s worthy of clinical application.
